C Section question...

When do you find out if you will have a csection vs vaginal birth.

I was asking the nurse at my ob's about the child birth classes today and she said....

"Your having twins right? Aren't you having a c-section?"

I know things can change, but right now both babies are head down and doing well. (knock on wood). I guess I was surprised, but what she said. My OB said it is all based on their position at my first appt at 9 weeks.
TTC since May/June 2012

SA #1 Low motility (17%) 
SA #2 Lower sperm count, but much higher motility
CD 3 Bloodwork and ultrasound - Normal HSG - Clear tubes 
Oct/Nov 2013 Cycle 1 with RE 50 mg Clomid + Ovidrel + IUI BFN

  • I'm hoping to have a vaginal birth, but I have thought it will depend on their positions once I get closer to.  My friend had one twin that was breech the whole end of her pregnancy, so they were pretty set on a c-section.  My OB hasn't said one way or another at this point for me.

  • I was given the option with our twins.  IF both babies are head down, I am welcome to try vaginally.  Going by your OB's comment about 9 weeks, A was head down and B was breech and has been until just three weeks ago.  Some crazy shit has been happening in my belly, so it wouldn't surprise me if someone was flipped again.

    Considering the risk of Baby B flipping after Baby A is delivered, we are choosing to have a C-Section.  We will be letting our OB know this on Monday.

    I guess they will schedule it then?  According to gestational averages for twins, they'd be born around January 13, even though my due date is February 9.  No clue how they will determine when it should happen......

  • I have a few friends that have twins.  Some have gone on to vaginal births and some with C-sections.  The ones that had c-sections were due to position.  One friend was breech and one was transverse.  I also had a friend that the babies were too big (each twin was over 8 lbs !) couldn't imagine. Glad to hear your girls are head down, FX they stay that way for you.

  • With twins it's obviously different, but I would think as long as they're both in favorable positions for delivery you should be able to try a vaginal delivery if you want to.

    Even with a singleton, you sometimes don't know what type of delivery you will have until the time comes.

  • The two ladies with twins from my last BMB were both able to have vaginal births. I know for at least one of the ladies, her OB turned baby B after baby A was delivered. 

    I know it's different with twins, but I would be surprised if their position at 9 weeks really determines that. So much can change from then on!

  • No input on the c-section, but this little one was head down at around 15ish weeks per my OB. At my last appt, he did a quick u/s to check my placenta and she was transverse. I don't know if it's common or not, or normal to change positions, but I am hoping she is head down come delivery time.

  • My BFF delivered hers vaginally at 36 weeks (she was induced) and I think it was just wait and see until then--they were in good position but keeping a close eye on her fluids and if she went before that I think they would have taken them via c-section. But she delivered in an OR and they *almost* took B via c-section after she delivered A, but they were able to move her down the canal successfully with a little vacuum. Definitely have the conversation with your OB, and I assume you can keep having the conversation until you can't, kwim? Hopefully it all works out the way you want it!!!

    I'm not a twin mom, but my c-section was unplanned, like many others. I have a few twin mom friends who have had vaginal births. Their kids were in good positions and their OBs let them go. If you are really hoping for a vaginal birth, let your OB know how you are feeling. Maybe they can put off making the call until you go into active labor.

  • Not delivering twins but my doctor said not to worry about position until 36 weeks and we'll decide from there. My last was a c section due to breech presentation and I'm hoping for a vbac this time. I would talk with your doctor about how you want to deliver and go from there, I never planned a c section but Dd had her own plans.

  • I personally am having my 3rd csection. It was scheduled at 24 weeks for when I am 28 weeks. I dont know about twins, but your baby can be upside down the week before and they wont just schedule a csection for it - there are methods to get a singleton to flip into correct position that they would due vs just doing a csection - but this is my drs practice.

  • I work in l&d and our doctors encourage vaginal birth if the first one is vertex. I have seen them flip the second one after "a" is out an I've seen them deliver "b" breech as well. The thing with twins is there has to be more intense monitoring and consistent cervical change. We have had more vaginal twin births than c/s. ( our hospital is the lowest our state at 20% for overall c/s rate) It comes to what you and your provider are comfortable with. Good luck!
  • My OB had said that if both babies were vertex, we would try vaginal. I ended up having complications with Baby A's growth so in the end I did have a c/s even though they were both head down. I found out when my MFM recommended early delivery that he also recommended a c/s. My OB agreed with him.

    This time, I am hoping for a VBAC. Right now, my obstacle is marginal previa so I'll find out around 32w if that's resolved.

    My Baby A has been head down this whole time & my baby B has been breech for the last like 10w. If they stay, I decided I'm opting for an C/S. If they both turn head down and stay.. I'll try vaginal. My OB/MFM hasn't even said anything about it yet.

    Edit to add: My OB/MFM has said they will vaginal if Baby B is breech & of similar size... I'm the one who would choose not to.

    Thank you everyone for the feedback. I think I was just shocked the nurse assumed I would gave a c section just due to twins.

    Next appt I will make sure I bring this up! The glucose screening had me feel yucky, so I wasn't asking a lot of questions yesterday.

    I am not opposed to a c section if it is the safest, but I would like to try a vaginal birth if I am able to. The one thing I know I don't want is the double whammy!
