Just found out I am pregnant with my third (his fourth). My boys are 9 and 6 and my stepson 15. This was very much a huge surprise as I was told 6 years ago i could never have any more kids. I am also 36 now, so in addition to my rare condition I am also advanced age.
What "extra" happens compared to when I was younger with my others?

i was 38 when my first was born and will be 40 when #2 arrives in December 2014. with the exception of being offered all of the genetic screenings available, and also being told very clearly that they are optional, i have never been made to feel "old." my age hasn't been a factor in any of our conversations with care providers (in fact, it rarely comes up at all). the whole advanced maternal age thing seems a bit outdated to me, to be honest. most of the women i know who are pregnant or have young children were over 35 when their kids arrived, and age just wasn't considered an issue.
you may feel a bit more tired this time 'round, but it's hard to know if it's your age or the fact that you have three older children you're raising. hard to know. at 40 i am having a totally normal pregnancy and feel great. i certainly don't feel old in any way.
Congrats! I have not had any "extra's" my Dr. is pretty old school. He does not even do a lot of the newer tests. He has not even said anything about my age. I am 38. I am in good health and have had 3 healthy kids all ready, so maybe that has something to do with it.
