Re: Weekly GD Check In - 08/26/14

  • Going pretty well.  The MFM hasn't increased my NPH dosage in a week and a half, as the fastings are finally mostly under 95.  Moving to weekly growth checks next week.   However, as Schroeder is doing well and it measuring just above 50% percentile in gestational size, they seem happy to let me stay pregnant until 40 weeks as long as she continues to do well (I even made a 40 week growth check last week).

QOTW:  I adore chocolate covered gummy bears and would love to eat some.  Not the easiest food to find normally and not at all on the GD diet.

  • Hi, Im Katie. New to this board. Figured I'd finally check in. I am due October 29. Baby boy named James. :)

I was put on a gestational diabetes diet last Friday.

My breakfast carb allowance is between 30-45 carbs. I ate two pieces of toast for 26 g of carbs and 2 beef little smokies for 4 g of carbs for a total of my minimum carbs of 30, and got a 142 reading. I'm supposed to be 120 or below. I checked the nutrition facts before I ate and then again when I got my high number to double check and everything was fine. Does anyone know why this would spike my blood sugar? I haven't had a high number for breakfast yet, and having eaten the minimum required I'm super confused!

So far my only high numbers have been when I tried something pizza related. I had pizza rolls within my carbs range and a lean cuisine pizza and both spiked my blood sugar super high so I know to stay away from pizza stuff! But other than that and this morning my numbers and ketones have great results.

I could kill for like just one pretzel m&m or something though haha +/- 60 more days to go!

Thanks for any help! Look forward to joining in on the October 2014 mom boards finally!

  • The MFM has finally started raising my insulin dose to more effective levels! Yay! I have an appt with their office tomorrow for a growth scan, which will be exciting. I had one of my twice-weekly NSTs this morning, and it was beyond boring since I forgot my phone and couldn't mobile bump from the table.

I'd love to be able to eat French fries. I miss those. Any deep fried starch (or anything battered and deep fried) is the worst thing for my blood sugar, though. The fat and carb combination always shoots me up well into the 200s.

I've learned it's not as black and white as "stay below carb minimum and your blood sugar will be fine". Different foods do different things to your body at different times of day. For me personally, I could not eat 2 pieces of toast at breakfast. I can do one, plus 2 pieces of turkey sausage. But the only meal I can handle 2 pieces of bread is lunch. Maybe it is because you have been fasting while sleeping, and so your body handles it differently?

  • Check-in: I'm pissed. Had contractions last Thursday-Saturday after an 11 hour workday running around in the heat on Thursday. Was on bedrest for the weekend til they stopped, which they did. Now my doc says I really need to take it easy. I am back at work, but he said NO MORE WORKOUTS! AAAHHH this is seriously going to drive me insane. I've been doing an intense elliptical cardio workout plus weight training 4 times a week and it saves my sanity. I am very very worried that stopping the workouts is going to do horrible things for my GD. My numbers have always been great but I think working out had something to do with that. I'm also worried that both me and the kid are going to blow up in size. Anyone else had to stop workouts, and how did it effect your GD???

Ugh, I wanted to vomit last night when I faced another plate of food.  There was just nothing appetizing at all about it, and the fact that I can eat chicken and vegetables, then walk 2 miles and still be 103 is freaking me the hell out.  I'm going to have to search for new recipes.

Sorry all I do is whine.

I can also only tolerate a smaller amount of carbs at breakfast - my normal breakfast is one frozen multi grain waffle with peanut butter on it with two turkey sausage patties on the side.  The rest of the day I can tolerate a higher carb limit, but not until lunch.

Also, what type of bread?  Whole grains typically impact your sugar level less than refined grains (ie, whole wheat bread is better than white bread).

Whine away!  GD is confusing and makes no sense most of the time.

  • Thanks for the help! I'll try one piece of toast in the morning and see how that goes instead. I was surprised since I've had a breakfast burrito wrapped by a large tortilla and been fine.

I'm envious of those that still can have peanut butter. Theres a big no peanut butter sign on my food chart that the dietician gave me. :(

The only thing they approve for a sweet tooth is sugar free popsicles, sugar free jello/pudding, fruit and yogurt.

I can't complain too much yet since I only have to control it with a diet so far and not use insulin but this hit or miss business on somethings working and not others and wading through trying to figure it out is the pits sometimes.

  • No peanut butter period. I asked if there was any type of exceptions and I was told no. :/
  • The diet has been very challenging this week. Numbers are perfect and then just shoot up for no reason. MFM told me to start taking glyburide at lunch instead of bedtime which has lead to some not so fun crashes.

Had a crazy moment last night in the gas station trying to find something I could eat on the way home from the gym (walked 50 mins and then my sugar level tanked) and then went off to walmart to find better snack to avoid that the rest of the week since I have been instructed to walk daily for an hour.

Today my fasting was 98 and post-breakfast was 122 so today I am still high after the exercising, the diet, and the meds.

This week if it could be anything it would be Chinese food.  I have been craving it so hardcore but I just have to stay focused.  I literally got in a fight with DH about meal planning the other night because I didn't want to think about food and carb counts anymore that day.  He was feeling sick and he was zero help.  I fixed myself something without speaking to him, showered, and watch tv in our bed.

Once I had a nap and it was time for my snack I came out and pretended like the whole thing never happened.  He literally said "I don't know what you can eat" .  It took all I had not to tell him that he could count to 45 as good as I could.

The next day while I was at my shower, he went to the grocery store and got what we needed for dinners and snack this week and had dinner cooked last night.  So grateful for the help because it really gets tiring thinking about GD all the time.

    Apologise for the novel...

  • Check In: How is the diet going? Any challenges that we can help out with? Any new recipes/ successes that you want to share? Rant/Vent about the lack of ice cream?

Diet is going relatively well, OB is still pleased with my numbers, so that's good. I've had a lot more energy to do things lately, which I think has led me to doing way to much and today I feel like I got hit by a train :( I couldn't take it anymore the other day so DH took me to get my favorite frozen custard, I had a very small scoop and enjoyed every single bite!

  • No peanut butter period. I asked if there was any type of exceptions and I was told no. :/
Wow that is strict.  My dietician actually mentioned peanut (and other nut) butters as a good example of a balanced food that they encourage GD women to eat (carbs, protein, and fat).  Now, many of the nut butters that are marketed to kids do have a good bit of added sugar, as does the reduced fat version - so read the label carefully.

Several years ago, they did advise that all pregnant women stay away from peanut butter as they were afraid of having a peanut allergy develop in the fetus.  That has since been proven to be faulty science.

Also, surprised at the severe restriction on sweets.  I know many of us use ice cream as part of our night time snack.

I've been pretty happy how well this week went as far as my numbers. I really watched what i was eating and got strict with myself again. I did find some new snacks that have helped me out too. I am a Pumpkin Spice Latte lover, today was the first day starbucks was selling them. I planned my breakfast accordingly and drank a tall PSL; 2 hours after my numbers were at 74. I'm taking that as a win! I probably wont have another until the baby is here, but it was such a nice treat!

QOTW:    If you could eat one food and it not impact your blood sugar, what would it be?

Homemade chocolate cake, with homemade chocolate frosting and maybe a slight dab of ice cream with toasted coconut on top! I'll just go daydream about that now....

  • Check In:   Going out to dinner to a non-chain restaurant threw off my numbers, but I can understand why looking back.  I'm struggling with breakfast.  I made a smoothie the other morning with frozen strawberries and blueberries, almond milk, and some greek yogurt.  I could barely eat it.  No sweetness at all.  Also, loving the ice cream before bed.  Makes GD so much easier to tolerate!

QOTW:   I have a recipe for egg-free cookie dough I love to make, but there's no way I could eat it now :(

Who taught your class (nurse at your OB's office, dietician, nutritionist, etc)?

Yeah that's what a few people I know that had gestational diabetes tell me, that my plan is super strict. My friend was able to have peanut butter and atkins candies etc and it was fine. The dietician at the endocronoligist office that I was referred to by my OB is who gave me the strict diet, and it wasn't one personalized for me but one they give to everyone they see.

I was also told my carb count they have me on was high. Breakfast is 30-45 carbs, Morning Snack is 15 carbs, Lunch is 45-60 carbs, Afternoon Snack is 15 carbs, Dinner is 45-60 carbs and Bedtime Snack is 45 carbs.

  • @golfingdarwinfish; I've learned (and also the basis for GD diet Protein v. Carbs) that the more protein I have in my meal, the better my numbers are. So for breakfast I ate a very bland breakfast consisting of 2 eggs, an English muffin size sausage patty and my lovely latte.  I am allowed 30g of carbs in the morning for breakfast. In the tall pumpkin spice latte there are 39g of carbs and 11g of protein. I knew I would be taking a risk, but this time it paid off. I also was sure to drink a few cups of water in between eating and drinking the latte.
Hope this helps and you're able to enjoy a latte here and there!

You need to get a personalized plan based on your weight, activity level, and expected weight gain - this should be done by a nutritionist or dietician.  A generic one is good as an outline to get started while you wait for that appointment but even this one is higher than the generic ones online that I found.

As an example, my initial limits were:  30 for breakfast (seems to be the one standard for most people), 30 for morning snack, 45-60 for lunch, 30 for afternoon snack, 45-60 for dinner, and 30 for evening snack.

In reality, I have found this works better for me:   I eat 15 -20 for breakfast, 0 for morning snack (due thyroid meds, I don't get breakfast until 8:30 and it is too hard to fit in a carby snack and still have good lunch numbers), 45 - 50 for lunch, 30 for afternoon snack 1, 15-30 for afternoon snack 2, 45-50 for dinner, and 30 for bedtime snack (sometimes higher if I have a low number after dinner).  I have two afternoon snacks as lunch is typically 12:30 and dinner isn't until 7 pm.  So the carbs from the missing morning snack got moved to to a second afternoon snack to keep my sugar levels more stable (however, I get a protein snack still mid morning).

  • Hi everyone, I'm new here, although I have been keeping up with these weekly threads for the last month or so.

My numbers were good at first, but then as the pregnancy as progressed, my numbers just kept going up (mainly my fasting and breakfast numbers) so they put me on GylBruide last week.

I've been taking 1.25mg every night at bedtime so help with fasting numbers but breakfast numbers were still in the 150s-160s so they emailed me yesterday to tell me to take an additional pill 30-60 mins before breakfast. its starting to help. but I noticed that my lunch and dinner numbers were on the low side. they used to be in the 100-115 range but today it was 79 after both meals. (I test 1 hr post meal btw and they want it to be under 140) Is 79 an acceptable number, or too low?

I have my regular apt with my OB tomorrow morning first thing and I guess I'll ask her what the plan is for delivery...I've read that sometimes doctors will induce at 39 weeks but some let you go to 40.

I also have my first NST tomorrow morning right after my reg OB appt. wish me luck.

If I could have any food it would be Swedish fish and gummy bears. I'm a candy whore.

Diet is going ok. I have been struggling with what to order when we go out to eat. I have taken some risks that haven't always worked out so well and other times they are fine. My fasting numbers in the morning are still all over the place. I meet with the nutritionist again tomorrow and I know she and the endocrinologist are going to increase the insulin again tomorrow.

I have had 2 NST and baby girl was not responsive in either of them so I changed the time to the AM. She is fine during the u/s (i've had 3 in a week and a half) so I am hoping it is just a time thing. The MFM told me last week she will likely want me induced by 39 weeks if I don't go before. I have been enjoying this pregnancy and I feel great still, so thus is just frustrating me. Sorry for the rant.

  • My nutritionist said she didn't get concerned unless numbers dropped below 60. Personally I feel shitty around 65. They may change the timing of your dosage. They changed mine from bedtime to lunch time and i hate it. I will talk to MFM on Thursday about it.
  • The NP I saw has some really low numbers for blood sugar guidelines. 2 hours after meals cannot be over 105, etc. Anyone else? When I had seen my Endo before he told me up to 120 was in range. I'm having a hard time keeping numbers so low. She made it sound like I'd have to go on insulin if I go over...seems harsh

Food: That would be everything. Oh, pick one thing. Let's see: sushi is probably at the top of my list. I had to give it up pre-pregnancy just to keep my A1C in check. 1 slice of a roll is 10 grams of carbs. It makes me sad.

They have a roll here called lady Linda. It is crab meat rolled in cucumber, no rice with a light watery sauce

    It's really good

  • With glyburide I have crashed a few times. You go from fine to really bad very quick

I have found if I hit 70 than I better act quick because 1. It seems harder to stay in range the rest of the day and I yo yo and 2. Drops happen very quickly like 15 mins to 30's and 40's. I passed out at 36 at work the other day. I felt fine and then in a matter of minutes I felt drunk and was out. It had been 119 an hour before that.

I initially thought, hey wow, that is pretty aggressive. But then I re-read that you are checking two hours post prandial and reconsidered. While I think a BG of 120 is fine, that's typically the highest point some offices want you to be after ONE hour post prandial. So at two hours, technically you should be coming down. Have you tried checking at 1 hour just for data collecting purposes? And how far out of range are you from the 105 goal?
My endo & nutritionist both want 120 2-hours post meal.  I had no idea that would be considered high by some!

I have an appointment with my endo shortly, and I have a feeling we are going to discuss medication, at least for fasting numbers.  I'm not really comfortable with glyburide, so not looking forward to that convo.

@starla487, I have always heard that you should be no higher than 140 one hour post prandial and no higher than 120 one hour post prandial. I found a link on the American Diabetes Association website with the same information.

That's sort of what I am hoping happens as after performing a literature search on glyburide I would prefer to skip it.  My MFM thought my high morning numbers may b

This! My endo said that OB's love to prescript glyburide, but she only does insulin. For my GD, I trust the endo more.
Oh good, I am really hoping I don't have to bring out the glyb trial data during this appointment. My endo Is still an academic, so I'm hopeful she'll be on the insulin train. FWIW, my MFM also prefers insulin, and they may have already had that convo.

No issue with glyb; my endo doesn't use it in GD.  She exclusively uses  insulin.  I'm supposed to increase my carbs because she thinks my current levels are going to end up causing issues, and I'm going to take 6 units of insulin before bed in an attempt to help with my fasting level, which she said is probably the most susceptible to the hormones being secreted by the placenta.  A1C was 4.9!

The carb increase may be a bit difficult, but she said she's open to having a call with my therapist, nutritionist, and MFM to discuss how to approach the increase.

  • @golfingdarwinfish I hope you have great results with the insulin. I have been very pleased with it, especially after being on glyburide for a few months.

I saw my MFM this afternoon, and he says the baby looks good enough that I shouldn't need to come back for another ultrasound! The Boy measured at the 59th percentile, with an estimated weight of 5lbs 9oz already. That's more than I weighed at birth. We also talked today about induction, and the MFM mentioned that one of the OBs I frequently see at my practice likes to induce at 38 weeks. I think I'll try to make my subsequent appointments with one of the OBs who is willing to wait a little longer.

  • I'm starting on week 3 of monitoring my carbs and my numbers are great! I'm so ready for this all to be over so I can go back to eating a somewhat normal diet though.  Thank you to whoever posted a few weeks ago about the Snickers Ice Cream bars.  I have been having those as my evening snack, and it has helped to satisfy my sweet tooth.  I meet with a perinatologist on Friday for an ultrasound and to check my numbers.  I'm hoping that he will say the baby is growing right on track, and I won't have to deliver early.

I made a "Baked Potato" Soup last night using cauliflower instead of potatoes.  It was quite delicious, and I couldn't really tell the difference except for the occasional chunk of cauliflower (I should have used the food processor to chop it up real fine).

Right now, I can't think of just one food I would like to have.  It seems like the moment you are told to stop eating something specific, you want it.  I could go for a big glass of ice cold sweet tea, some moose tracks ice cream, a big bowl of cereal, a bag of potato chips, and the list could go on.

@golfingdarwinfish‌ If you were put on NPH, I'd strongly recommend a 2 am check now. (Just solely based on my issues with it.) At least for a couple nights at first. Good luck. Kudos on the kick ass A1C.
    @starla487

I'm going to start the humulin tomorrow, and will definitely do an am check.  At first she wanted to start me on 3 units because my fasting numbers are only occasionally over (peaked at 105 and 95 is her cutoff), but I think her hope is that this also takes the edge off a bit regarding my number obsession as the high am number will greatly impact how much food I allow myself during the day.

Did the NPH bring you dramatic lows?

  • Diet is going really well for the most part. I haven't had 1 blood sugar out of range this week! Makes me happy! Hopefully I (and the glyburide) can keep it up!

That was me! I have also had good experiences with the Twix ice cream bars and single serve Haagen Dazs ice cream cups. If I'm craving chocolate, I will have 2 pieces and a cup of soy milk.

That cauliflower soup sounds interesting - I have also heard of cauliflower rice and risotto but haven't tried that yet.

Dinner last night may or may not have been a Magnum ice cream bar, 0% Fage, and broccoli...  I'm so bad at feeding myself when I'm home alone.

And my number may or may not have been 100 afterwards.  :)

  • I'm so jealous about the ice cream! It's one of my most missed foods but my dietician put ice cream under the 'no no' list on my plan. Is this the case for everyone and you just kind of sneak it because it doesn't affect your numbers or were you given the go ahead? I'm really tempted to try it out. This diet sucks..

Look on the bright side - lots of calcium.  And the yogurt and ice cream have protein.

Add in a spoonful of peanut butter or two and it sounds like some of my dinners when I don't want to cook!

Exactly!  I only ate the yogurt because it has 23g of protein and I thought it would help counteract the ice cream.
