IUI to IVF decision makers

So after IUI #2 and a BFN, we are taking a cycle off (giving it the good old-fashioned natural way). We will do IUI #3 in August. After that, we have to decide whether to move ahead with IVF (forced break due to finances). My question is: how many IUI cycles have you tried before moving to IVF?

  • OP it looks like we are in the same boat. We had two failed IUIs. We are taking a break this cycle and we will try another IUI in August. I will also start seeing a new RE so maybe his protocol will do the trick. But if this next IUI doesn't work we will start to look into IVF.
    Me: +35 DH: +35
    TTC: Since January 2013 
    DX: PCOS. Severe Endometriosis, Unicornuate Uterus w/only left tube and left ovary, Pedunculated fibroid (on the outside of uterus) and Anovulation. All conditions diagnosed 8/13
    TX: Metformin
    DH DX: MFI - low morphology, low motility
    Ultrasound shows both kidneys in spite of UU. 
    HSG showed clear tube on the left side. 
    Lap Surgery performed 1/9/14 to remove fibroid and endo (Stage 3)
    • IUI# 1 June 2014 started 100 mg of Clomid - 7dpiui Progesterone: 13: BFN
    • IUI#2  July 2014 started 100 mg of Clomid - 7dpiui Progesterone: 5.75: BFN
    • Natural Cycle - so shocked to be in 2WW - 7dpo Progesterone: 15.5: BFN
    • Working with new RE starting injectables in late August.
    • IUI #3 August 2014  w/ Menopur: BFN
    • Finally ovulating on my own!!
    Waiting to start IVF hopefully

  • We did three IUIs. I ended up having a fourth cycle that was going to be IUI and was cancelled, but that was being done "in the meantime" while we prepped for all the IVF stuff we had to do.

    Me: 32  DH: 33  Married: March 2004

    July 2006: started TTC
    2008: HSG (normal), couple rounds of clomid through gyno
    2008 - 2010: dragging my feet out of fear and procrastination
    October 2010: first consultation with RE, dx PCOS and fibroids (DH slightly low count/motility)
    Oct. 2010 - Dec. 2012:  In DENIAL! avoided the issue because I was scared of surgery
    January 2013: returned to RE, fibroids grew significantly
    February 2013: second HSG, fibroids pushed on tubes which blocked them somewhat
    March 2013: MRI to determine what type of surgery may be necessary

    July 29, 2013: fibroids (5) removed via robotic laparascopy
    August 2013 - Nov 2013 : benched due to recent surgery

    IUI #1, Dec. 24, 2013, BFN 
    IUI #2, Jan. 25, 2014, BFN
    IUI #3, Feb. 25, 2014  BFN
    IUI #4 canceled due to lack of response to letrozole
    IUI #4.1 April 28, 2014, BFN

    May 16, 2014: wtf consult, start prepping for IVF in June and add injects for one last IUI in the meantime
    IUI #5 started letrozole and bravelle but canceled after HSG led to new diagnosis

    May 21, 2014: third HSG, tubes blocked, one at the beginning, one hydrosalpinx??
    June 11, 2014: consult, approved to move on to IVF because the hydro is not completely blocked therefore allowing fluid to move through slowly rather than backwards
    IVF #1 August 8, 2014 - 3dt of 2 embryos, BFN
    September 17, 2014 - 4th HSG, the right tube is very patent (open!!) dye went straight through this time. Weird!
    October 2, 2014 - started metformin treatment
    November 14, 2014 - blood work, brought A1C down from 5.8 to 5.5

    November 26, 2014 - RE finally back from vaca and reviewed my chart, no more IVFs for rest of calendar year
    December 1, 2014 - Right after Thanksgiving, I called a new clinic and got in right away! Plan for IVF
    December 17, 2014 - ER! 29 retrieved (!!), 16 mature, all 16 fertilized (ICSI)

    IVF #2 December 20, 2014 - 3dt of 3 embryos, BFN

    We are done with treatment unsuccessfully. :(

  • We have had 4 IUIs and one cycle with meds and TI, we are just starting to prep for one final IUI before moving to IVF if we need to.

  • We were advised to not even try IUI because of Hs count after wash and we are OOP. Our options are at this point IVF and TI. We are opting for TI since we don't have the funds for IVF and we aren't ready emotionally for it. We are going back for more testing (blood & SA) later this year. Then we will discuss our options again.

  • I'm in a similar situation and have been struggling with the choice. Our RE thought we had good chances with iui but after 3 failed I'm not sure of the next move . I have a lifetime coverage of $15,000 for infertility treatments so I don't want to waste it on iui. I finially opted to do a final (4th) iui in August - mostly due to we have company and didn't want to start ivf process with ppl in town. If #4 is a bust I will be moving to ivf in the fall.   • Hi everyone, thanks so much for sharing all of your experiences. It sounds like many of us are in the same boat. There is definitely a component of feeling emotionally ready for IVF (in addition to the financial factor, because our insurance doesn't cover it). I'm keeping my fingers crossed and hopes up that August's cycle w/IUI will do the trick, but we'll see.

  • We are not completely positive, but I think we are moving to ivf after 2 iui's. We were going to try 3, but DH's post wash counts are not good.

    January 2010 SA results: Count 16 million, Motility 40%, Morphology 2%
    January 2010- Surprise BFP! DS born 10/1/2010 :)

    September 2013 Repeat SA: Count= 1.7 million, Motility= 24%, Morphology= 2%
    November 6th 1st Appointment with RE: diagnosed with severe MFI
    Testing to try to determine a cause & possible treatment for MFI
    CD 3 blood work for me. RE does not want to repeat my HSG/lap at this point,
    but may want to before moving forward with any fertility treatments.
    After seeing the uro, DH is currently taking lots of supplements and clomid to try to boost his count. We will have a repeat SA in February to see if it works.
    Follow up SA numbers are: Count= 4 million, Motility= 40%, Morphology= 1%
    Uro wants us to have another follow up SA 5/9 to see if we see further improvement than we are back to the RE to make a game plan.

    SA 5/9/2014 Count: 12 Million, Motility: 60%, and Morphology 2%. We will be doing iui #1 in late June

    IUI #1 6/28 clomid + ovidrel, post wash count 3 million total sperm= BFN

    IUI # 2 7/21 clomid+ ovidrel. post wash count 900,000 total sperm= BFN

    IVF planned for early November- cancelled due to cyst

    December IVF #1- 22 eggs, 20 mature,16 fertilized

    12/9 Transferred 1 4AA Blast, 6 frosties

  • We did 3 IUIs, 2 with clomid and 1 with follistim.  Our RE pretty much said from the get go, that we would do 3 and if that didn't work our best bet was just to move forward with IVF.

  • Pretty much sounds like 3 is the magic number. We tried this cycle naturally, so I'm hoping it just works so I don't have to think about next steps...LOL a girl can dream, can't she?! :)

  • Looks like I'm outside the norm here, but I'm on IUI #6 (2WW). The first one worked, but ended in a loss, so we figured we'd keep trying and it was bound to work again. It's really affordable with my insurance, so we're doing as many as they'll let us. All have been with Clomid, but this was the last try with that and we'll be switching to Letrozole if this one doesn't work.

  • @BKBABY3‌ so sorry for your loss. We have 6 cycles of IUI covered by insurance. However, they told us that the we can use the meds. for the remaining 3 cycles (if we don't do IUI) towards our IVF cycles. Finances are not driving our entire decision but they are definitely playing a part. It sounds like your insurance covers a lot more. Good luck!

  • @clover30, yes my insurance is actually pretty awesome when it comes to IUIs. However, I have no coverage whatsoever for IVF, so I'm very motivated to exhaust the IUI option first (hence #6 and maybe #7 after this)!

  • On a related note, when do you have an IVF consult? Is it when you decide to move on to IVF or just when you want to know how it would work and what the procedure/costs would be?

    IUI#2: abandoned... O'd early & DH hormone issues (June 2014; Letrozole 2.5 mg x5 days, Bravelle 75 IU x2 days)
    IUI#2.1: BFN (July 2014; Letrozole 2.5 mg x5 days, Bravelle 75 IU x4 days)

    Moving on to IVF. (Why we're moving on to IVF)

    IVF#1 (w/ICSI): BCP 9/9-9/23. Gonal-F, Ganirelix, Low-dose HCG (antagonist protocol). 41R/35M/32F... 2 transferred on 10/14, 14 frosties! On cabergoline to help avoid OHSS. BFN, possibly because of 90% drop in estrogen and progesterone a few days after ET.
    FET#1: Transferring 2 on January 8. BFP! beta#1 (1/17): 408, beta#2 (1/20): 1310, first u/s scheduled 2/5

  • @tweetyknicks‌ At our office, we were able to talk w/ the Dr. about the IVF option when our last IUI didn't work. I imagine we would go back for a more specific IVF consult once we're ready to start that process.
