It could be any number of things. I've had various bouts of bleeding. I always have called my dr and she had always been able to reassure me by listening to the heartbeat or having an ultrasound. Glad you're going in for that reassurance that everything is okay! Thinking of you.
If you've been constipated, it's more than likely bleeding from your cervix. I have had a 'sensitive cervix' this entire pregnancy. In fact, this exact same thing happened to me on Saturday. Had a "straining" BM, went to wipe, lots of blood in the toilet...discovered it was coming from vagina. I panicked for a quick second, then remembered my doctor saying my cervix was very sensitive. The straining can cause those blood vessels in your cervix to pop. Quickly found baby's HB with my doppler and haven't had any spotting or bleeding since then.

I just had this Tuesday for the first time myself. They did a urinalysis and a swab test, plus viewed the baby with an u/s and used a doppler. She did notced the cervix was low lying, which is common earlier in pregnancy while your uterus is still small, often growing out of the way as the uterus expands (had the same thing with my first, and all was fine). Doc alluded to the fact that that can cause you to see a little bleeding sometimes. May very well be nothing to worry about, which is of course what we're all hoping for! Best of luck!
Edit: Doc who saw me (not my normal midwife) suggested pelvic rest until my anatomy scan in a few weeks.
