I'm hoping @Skeemer has something diabolical to make him think he's going crazy.
Do you want to annoy the hell out of him, put him in his place/make him feel like the asshole he is, or something entirely more diabolical/evil but not illegal?
seriously - i'm not sure about the HR situation there, but I'd be first in line. THROW YOURSELF DOWN THE STAIRS?! WTF?! i'm a REALLY laid back person, and not much offends me, but that's so over the line you can't even pretend it is ok.
other than that, i have nothing to add - i'm always the "think of something really awesome to say later" kind of person. maybe once my brain has processed his douchebaggery...
I agree. If nothing is done I'd be going to local news media, but be careful of course because of libel/slander. I know you know that because you are smart, but it has to be said.
I would also create a paper trail if I could documenting every single incident.

Can you schedule a meeting and have HR dial in to the call, and have you express your concerns at that time? You could talk to HR beforehand to see if they think this is a good idea or not.
You still have a few months until you're leaving, so I think it would be a good idea if someone told him that his comments need to stop immediately. Whether that someone is you (with HR on the phone) or HR directly, I think he needs to hear it. He sounds too dense to realize this on his own.

@aliletz totally read my mind! I love these products! The annoy a tron nearly drove my boss totally crazy for a while. He rebooted his computer, he was tearing his office apart & it was totally AMAZING. When he left his office I took the annoy a tron out. Waited a few days & then would return it.
Also another awesome one is to sign him up for everything via email. Military recruiters, Mary Kay, Avon, Tupperware, Premier Jewelry...whatever "contact me" you can think of...sign up his work email & throw his cell number in there too.
Sign him up for mail order catalogs (yes, those still exist) too. It's even better if it's sassy or etc. Like him getting catalogs for Victoria's Secret, Fredricks of Hollywood, & the likes. Wait a couple of weeks & all the catalogs will start filtering in to work. With his name on it. He's going bald? I'm sure you can find some toupee/wig catalogs to make their way to him.
Seriously ten minutes with this link & the fun will never end. He'll be getting stupid catalogs until the day he dies.

These stink bombs are nasty. They don't last all day but will give you a nice half hour of him grossing out. For best results break it on a smooth service (such as the corner of a tile floor office) because the smell will linger longer.

I love Thinkgeek, have made many purchases from there over the years. I have the annoy-a-tron and it was so much fun back when I was in grad school (I used it so much it stopped working)! I bought the air cannon and gave it to my sister, she uses it to keep her cats off the kitchen table (a spray bottle of water was no longer working) and to annoy her DH from time to time.
I wouldn't do anything at his level (even though it would feel awesome) because HR is much less likely to take your complaint seriously then. Be completely professional and let him look like the asshole he is. He'll do himself in.
I get the impression you were completely offended, and in that case, I'd file a formal complaint with HR. As an HR rep, I can tell you though that as soon as you take it upon yourself to seek revenge, it really complicates your case. In retaliation, he would be quick to report anything you do---guaranteed.
It's not worth it to risk losing any potential payouts when you leave, or a positive reference check when you get back into the workforce at some point, nawmsayin?
