Help! Provera and Positive OPKs!

Wednesday, January 29-
So I am on Cycle Day 39. My doctor gave me a prescription for Provera a few days ago after confirming that I did not ovulate last cycle and that I was not pregnant. The Provera is 10mgs once day. I took the first one last night and about 15 minutes later I noticed I had EWCM went I went the restroom...I also started having ovarian twinges on one side, light cramps, and backache over the course of the evening. I took an OPK and low and behold it was WAYYYYY positive. So then I took a digital OPK and it was positive. Today, I woke up with a headache (as I normally do when O is occuring) and I feel full (as I normally do when O is occuring). Took another OPK and bam! still positives!!!

My doctors office is not open today and there is not a nurse's line for questions. So I don't know what to do....Do I take another Provera tablet today as she prescribed?? If I take it will it prevent O from actually happening? We BD'd this morning in case O does in fact occur today. But now I am at a loss on what to do?? Help!!! Anyone else ever have anything like this happen!?

PS- this is the only discussion I've found on it really>https://forums.thebump.com/discussion/4709999/provera-can-make-you-ovulate

Thursday, January 30-
I used 2 different OPKs last night and they were both positive. I used 2 kinds again this morning and they were negative. Also, I did not take Day 2 of Provera last night since I hadnt spoken with the doctor. I talked to my doctor first thing this morning, and she said obviously to not take the Provera. I am gonna have b/w done to confirm that I did ovulate and then we will wait out my luteal phase for either Aunt Flo or +HPT. If b/w says I didn't ovulate then I will proceed with Provera at that time.

I don't know. That's tough. I think I would probably just continue to take it. I would be worried that I would screw up my cycle even more if I started taking it and then stopped.

I know it doesn't help now, but in the future you might want to wait longer to take the Provera to give yourself a chance to O late first. I know sometimes doctors have reasons for wanting you to take it sooner than CD 60 though, so I guess that is a decision you and your doctor have to make.

FWIW, when I start taking Provera, I stop doing OPKs and stop temping, so that I don't put myself in a situation like this where I am agonizing over whether or now I was actually about to ovulate, or if I somehow ovulated right when I was starting Provera, and that's why my temps might be up. I feel like, for me anyways, once I make the decision to start Provera, I need to just consider that cycle a loss and move on.

Good luck with whatever you decide.

  • I agree with @Slappaclicious, a 39 day cycle is pretty early for Provera. I just came off Provera a few weeks ago after a 70+ day cycle, and it's meant to raise your progesterone enough that when you go off it it created a withdrawal bleed. I don't see how it could possibly induce or cause ovulation.  Plus, a positive OPK doesn't mean you will ovulate for certain even if you stop the Provera.

There has to be someone covering your doctor's office, whether it be you Dr on one on call. I would try to get ahold of them and askl if you should continue.   Good Luck!

  • Thanks for the replies ladies....
Just for a little more background, I am on my 5th round of Clomid. I have O'd every other round between CD18 and CD21. I have very irregular cycles when I am not on Clomid. Normal cycle for me is like 180+ days long. I am not over or under weight. I have all normal levels of everything ever tested...and it is so far unexplained why I do not ovulate regularly.
FWIW, when I start taking Provera, I stop doing OPKs and stop temping, so that I don't put myself in a situation like this

I didn't temp yesterday or today after making the decision to take it...but every other sign felt/acted like ovulation....and all of this was happening within a little while after swallowing the pill....so i knew it wasnt the pill. So of course I had to POAS, ya know? And now that I have a positive, yep it has me confused.

kwicks, i understand its supposed to create the withdrawal bleed. I took it last year before i began clomid to start a new cycle. But I don't think its the Provera that is inducing ovulation....i think i mustve been setting up for it before taking that pill. Also I understand a + OPK doesnt mean for sure I will ovulate, but everytime I have taken clomid so far I've gotten a + OPK the day before I O'd. Also, my doc and nurse are both unavailable today...its a one lady practice....the phone lines say call back tomorrow or call 911 if its a medical emergency....its like this every Wednesday....and wouldnt you know my questions are always on Wednesdays.
  • I swore up and down that I Od due to pos OPKs, EWCM, and temp spiking.  I had a progesterone test with my RE the day after, and it said I did not O.  I was prescribed Provera.  I was hesitant to take it because I was certain that I Od, and we had good timing.  I am taking the Provera because my doctor's tests are way more accurate than I am despite the fact that FF is trying to convince me that I should be in the 2WW.

If your doctor's test said that you did not O when the medication was prescribed, and you began taking it, I would continue to take it.
  • If your doctor's test said that you did not O when the medication was prescribed, and you began taking it, I would continue to take it.
My doctor had me take an at home OPK and at home HPT before prescribing....not in office b/w. Reason being is that I've been able to pinpoint my O every other cycle and then call her and tell her when to do the b/w. This cycle I know I didn't so she just said do an OPK and HPT before taking it.
  • Wait, so what kind of monitoring are you on with your Clomid?
You should be going in for Ultrasounds because at home methods are not reliable enough when you're on drugs such as Clomid.

What kind of doctor are you seeing?  An RE?

If you didn't respond after the first two or three rounds, WHY didn't your (hopefully) RE give you a different protocol?

Was he/she hoping to use up ALL 6 of your allowed lifetime Clomid cycles?
After I ovulate, I go in for b/w to confirm I did ovulate. I have responded great to Clomid. I have O'd every cycle between CD 18 and CD21 as confirmed by the bloodwork.

Also, it's not 6 "lifetime" rounds of Clomid....I know lots of people who concieved on Clomid after rounds 5 or 6....and then a year or so later did more rounds to concieve again.

Either way....at least I found that I am not the only this has happened too....I found this post:

If you think you are ovulating naturally, I would not take it.  You can call your doctor to confirm later this week, but you should not take it during pregnancy which I'd think would include anything from conception on!!

Also, side note (to @slappalicious point above):  If you are to the point of taking Clomid, I would also seek the help of a Reproductive Endocronologist (RE), instead of a OBGYN.  REs specialize in getting women pregnant and know a lot more about the drugs and methods you should use to monitor women while trying to get pregnant.  Taking clomid or other drugs to ovulate can be dangerous without monitoring, because it increases risk for multiples, etc!!
