Husband is never required to attend, but is always welcome to come along. There's not much to interest him outside of ultrasounds (and honestly my husband isn't terribly interested in ultrasounds either) but if you need/want the support of having someone there with you- he'd be a good choice
You will fill out a family history form, talk about the pregnancy (any issues or concerns you or your doc may have), possibly a pap, pee in a cup, they'll take your blood pressure, check your weight, you'll get a script for blood work or do the blood work in the office (I get to go downstairs to labcorp), and you'll probably get a bunch of stuff to read, and maybe some samples of prenatals.
You might also meet with the financial people to discuss your payment method (insurance coverage or self pay- I've wound up in the coordinators office both ways)
Mine took an hour or so- but it was very boring stuff.

My husband went to every single appointment and didn't want to miss any. He has a son with another woman and she didn't tell him she was pregnant till she was 7 months along and even then wasn't sure who was the father so he didn't get to have any part in the pregnancy. He felt he was more connected to the baby being able to see the u/s and hear the hb for the first time, it was also nice having him be there for me, if I had questions I forgot he would ask for me. It's definitely a personal thing, if you feel that you want to and he wants to go, do it.
My timeline started a bit different due to previous losses so I had an early ultrasound, however at my OB the first appt (6-8 weeks) is basically family history, Q&A, urinalysis, blood work. Hubby goes with me for all of my OB appointments, but it's up to you two. At the 12w appointment they do an internal/pap and listen to the heartbeat with the Doppler. I guess it depends on how far you are, as everyone else mentioned and your OB office

It probably depends on how far along you are. My first appointments have all been between 12-14 weeks and they did a pap smear if I was due, urine sample, blood work, checked for hb with Doppler, and then spent time talking and getting a history. My practice doesn't offer early u/s. My husband has gone to some and not gone to others. With my 1st baby he went to almost every appointment but since then he goes to some here and there if they happen to be convenient but generally I go alone.
