Pregnant after IF

Yolk Sac & Beta Levels

I had my second ultrasound this morning (at 5w2d) and our RE said she saw what might be the beginnings of yolk sac, but nothing definitive. She was surprised because she thought at my beta level she would see something more definitive (my last beta was two days ago and came in at 1157).  She then said with beta that high she might suspect multiples but didn't see anything else in the uterus .So there might be something in one of my tubes but couldn't find anything.  If there is not another little bean out there, and they don't see a definitive yolk sac soon, then RE says that it is likely not a viable pregnancy.  I feel like I was hit with a mac-truck of information and am having a hard time processing through all of this. I don't know how I am supposed to wait until Monday for another scan.  I honestly didn't even think my beta was that high based on beta-base. Does anyone remember what their beta level was at when the RE was able to see a definitive yolk sac?

April 2014 - GonalF + Ovidrel + IUI + progesterone = chemical pregnancy :(

  • Many REs will not even do an ultrasound at 5w2d - its just way too early to really see things.  6weeks is usually the earliest they want so they can see things more clearly - some make people wait until 7 weeks even.   I would just remain hopeful and hang tight until your next appt - good luck!

  • My RE wouldn't give me an ultrasound until 6 weeks and 5 days for that very reason. It's so early and I'm sure they have to prepare you for the worst but I wouldn't count out your pregnancy just yet. Praying your embie sticks!

  • Thanks guys.  I am hoping you are right and it's just a little too early to tell.  Both DH and I are late bloomers (didn't get married till 34), so I'm sure bean is just following in his/her parent's footsteps.

    My RE doesn't do u/s until 6 weeks for that reason. Hopefully it was just too early. 

    My beta #1 was 1239 so I don't think yours is really that high at all.

  • I agre with PPs, my RE did an earlier ultrasound than he normally does, and that was at 5w6d. My betas are in my siggy, they weren't anything outrageous, esp for twins. Keeping FX for you!

    Me: MTHFR, Low AMH (1.1), High NK cells and Antiphospholipid Antibodies.
    DH:MFI-Low morph (1%), DNA fragmentation: Excellent!
    5 rounds of clomid, 3 IUIs, and multiple non-medicated cycles- BFFN                                                      
    IVF with ICSI in August 2013 brought us our babies.  ER-9R, 7M, 4F w/ICSI  ET of 2-Grade 2 blasts. 
    + HPT at 6dp5dt. #1 (8dp5dt)- 105    #2 (13dp5dt)- 510  #3 (15dp5dt)- 960  #4 (17dp5dt)- 1889
    Dx shortened cervix, PTL, and preeclampsia during pregnancy.  Lots of medications and 13 weeks of bedrest, babies were born healthy at 34w4d!

  • I had an early ultrasound to rule out another ectopic, and it was when I was 5w0d (it would have been 4w6d based in my lmp which is what they based my due date off). They also saw the beginnings of a yolk sac. My last beta had been taken four days prior and was 611. So it was probably around 2400+ they day of that ultrasound. And my little bean is doing just fine. So your numbers sound just fine to me. Fingers crossed all is well!

    August 16, 2013:  BFP our first month of "not trying!"  Still in shock.  Beta #1 (14dpo): 183.  Beta #2 (17dpo):  611.  Ultrasound 8/30/13: baby measured 6 weeks, 1 day, heart rate of 118 bpm! 
    Ultrasound 9/13/13:  8 weeks, heart rate of 176!
    Baby is due 4/26/14

  • Just to clarify, it was 5w0d based off the day I know I ovulated and conceived. They were of course stubborn and wanted to stick to basing everything off my lmp.

    Me: 42. DH: 46.

    My RE starts u/s one week after beta #2 (which would be approx. early in week 5 for everyone), and then does one every week after for 4 weeks.  My first one was at 5w1d and my beta that day was 7,630.  We saw a yolk sac and what looked like a little piece of rice, which was the beginning of what is now my 8w3d 1-inch bean!  Sounds like maybe your beta is still a little too low to see anything.  But I actually disagree with PPs that it's necessarily too early, 5w and a few days-ish is standard for my clinic and they do expect to at least see the gestational sac and the beginning of the yolk sac - I was lucky that I got to actually see the fetus starting to form that early.  Everyone is different, so they don't guarantee you that you'll see anything yet, but they do the u/s to check how things are progressing.  I wouldn't worry about it just yet.  Also, I see your previous beta levels, but what has your doubling rate been?

  • My RE wanted to wait until my betas were well above 1000 to do an ultrasound. When I went in for my US, it was almost a week after my last beta of close to 1900 and she saw just the beginnings of the yolk sac. It was tiny. I can't remember all the details, but I think I put them in my sig. Since you're just above 1000 and 5 weeks or so, I bet it's just too early. I wouldn't worry (easier said than done, I know!). Fingers crossed for you!

  • I had 2 sacs with yolks at 5w2d and my beta was well over 5000 by that point (first beta was 2,476 at 13dpt). If she saw the beginnings of one then there is certainly hope.  Don't count yourself out yet!

     I was doing a lot of research before my first ultrasound (which was 5 w 2 days). A yolk sac isn't expected until 5w 4 days. I think the early ultrasounds are used for REs to check to make sure the pregnancy is in the uterus. While my first ultrasound did show the yolk sac, it could be totally normal for it not to.

    As for concerns about an ectopic twin pregnancy, I wouldn't worry about it. I am really surprised that your RE is expressing concerns. Betas really aren't a good tell of whether your'e having twins. Mine was pretty high with this pregnancy (like in the twin/triplet range on betabase) but I just have one little bean in there.   • I don't know why your RE would say that to you...at 5w, a beta of 1100 is not that high.  Mine was 767 at 4w4d with 1 and as far as I know, it's healthy.  As far as I'm concerned, that was just mean.  Good Luck!!

  • Thanks so much for the kind words everyone. These posts are helping me to feel better and to have faith. Beta came back today at 1941 (two days after the 1157), so I think that's ok. Nothing to do know but wait until Monday and pray things are progressing the right way.
