  A baby carrier (I recommend the Ergo) if you don't already have one, its a must have for 2u2

  A second car seat (either now or when LO is ready for a convertible)

  New bottle nipples

  Extra washcloths (I go through a ton of them)

  Some new bibs

  Anything that you wish you had the first time- for me I got a 2nd boppy so I could keep one upstairs and downstairs

  Like PP we did another crib and DS2 ended up hating the bottles we had with DS1 (as well as the bottles that came with my pump) so make sure that you have a different kind of bottle or 2 on hand just in case.
  
  All the suggestions so far look great. If you work and use daycare I highly recommend a Snap n Go stroller with a large basket to get both LOs in. DS1 helps push and the basket is great for the days bottles, clothes, and biweekly packs of diapers. It's a time saver (everything in 1 trip) plus you're not wrestling with a toddler and an infant in a car seat at the same time.
  
  The only other thing I can think of is a bigger diaper bag.

  I forgot we needed a lot of new clothes even though I had another boy because they were born in totally different seasons. For example, DS2 is getting ready to hit 9 month clothes next month and I just realized all my 9 month clothes are shorts and cotton sleepers! Not going to work in November.

  Or just a second monitor camera.
