Delivering mo-di twins past 38 weeks

Here's the situation - I am 38w 2d with mo-di twins.  I went in for a scheduled induction last Thursday night.  I was 50% effaced but not dilated.  I was there for almost 3 days and after 1 round of Cervidil and 4 rounds of Cytotek I was still not dilated and was only 70% effaced.  The doctors were not keen on csection yet so they just sent me home this morning and told me to come back and we'll try again tomorrow night.  The thing I am worried about is that I have read that the it can actually be risky to deliver mo-di twins past 37 or 38 weeks due to placental breakdown.  I mentioned this to one of the doctors at one point yesterday and she just waved it off but there really are no MFMs down in L & D who specialize in twins (my normal MFM does not deliver in this hospital)  Anyway - I can't seem to find a lot of credible info on this online and I'm trying to gauge how much of a risk this really poses. If I thought it was a big risk, I might push the C section a little more forcefully with my doctor.

Has anyone delivered mo-di twins past 38 weeks?  How would they detect an aging placenta?  They had me on an NST - would they be able to tell from that?  I know most of you aren't doctors, but on a scale of 1 to 10 (1 being not a big deal, 10 being - I should be freaking out about this) how much of a risk is this when I've had an otherwise very healthy, complication free pregnancy?

  • I am 31 weeks with Mo-di twins.  We've had complications (developed TTTS and had fetal laser surgery to treat it successfully) so a bit different than yours.  I don't have much information for you, but for what it's worth, my perinatologists and regular OB have repeatedly told me there is no reason to leave my twins in longer than 37 weeks.  I've been told there can be more risk leaving them in than taking them out.  Twins are usually considered "full term" at 37 weeks. If I don't go into labor on my own prior, I have a C-section scheduled at 37 weeks.  Again, only my personal experience.  Sorry I can't be more helpful.  Good luck and good job!

    Can't you call your MFM and ask what they think? I have no experience in this yet but that would be what I would do.

    I will probably try tomorrow but its a long weekend so I don't think I'll be able to get hold of her.   She has an answering service but it can take awhile to get a response.

  • I would get your MFMs advice but honestly that would make me a bit nervous that nobody seems to be taking your concerns seriously. Placental breakdown can be signaled by fetal distress which you could probably see on see on a NST, poor growth and/or restricted blood flow. How often are they monitoring you? At this point, my non medical opinion is that those babies are better out than in. Too many risks at this point. Also huge congrats on going so far! I hope you have 2 healthy babies in your arms very soon.
  • I was told induction that far along with twins is typically NOT successful bc the uterus is too stretched out. If anything it would put the babies in more stress to continue trying to induce. I STRONGLY suggest calling your MFM in the morning and then have a FIRM talk with your DR. This kind of non valiant attitude is dangerous in my opinion.

  • Thanks for your advice guys. I'm going to try to reach my MFM today although I kind of doubt I'll get her on a holiday. But I'm scheduled in at L&D later this afternoon and I will be much more insistent with them this time. These girls need to get out!

  • My MFM was not going to let me go past 37 weeks, but ideally wanted them out around 36 because of the current medical research. I ended up delivering at 34 weeks, so I never dealt with this situation you're in. I hope that you are able to get in touch with your MFM today because I really think those sweet girls need to be out.

  • OK - couldn't reach my mfm but I called my regular OB who is luckily on-call in L & D today and he said come right in this morning, so I'm just waiting for DH to  get dressed.  My OB actually sounded a little put out that the on call doctor discharged me from L & D yesterday.  I think things will go better now that my doctor is the one in L & D.  He knows my concerns and my history  better than the doctors who were on call this weekend and will get me into c section today if it is needed.
