Baby N conceived after 1 miscarriage and more than 2 years of TTC. Diagnosis was low sperm count. We found success after 3 months of anastrozole to increase DH's testosterone and one IUI.

OMG, I can't even wrap my head around this! I'm sorry there is so much confusion and ambiguity for you right now.
So if the first pg isn't viable, will it negatively impact the twin pregnancy? Or will it just disappear?
I don't know. Hoping to get more answers after we can definitively say if that pregnancy is viable or not. Also planning a second opinion since I'm not trusting my RE all that much right now.
I believe what you are saying, but I don't think that is what happened.
If you were 13dpiui on Sunday, you'd be 17dpiui today. To see and diagnose identical twins via ultrasound at 17dpo would be impossible. So, you must be farther along than you think with those two. Since the bigger sac didnt have a heartbeat, it is either a blighted ovum, or not as far along as you think.
My best guess is that you are pregnant with spontaneous triplets from before your IUI (assuming they saw triplet sacs on the ultrasound... You weren't quite clear on that). Just because one is bigger than the other two doesnt mean superfetation. That actually happens commonly. Good luck at the next ultrasound and keep us updated!
He didnt diagnose identical twins for sure. Based on the size of the three sacs he saw one was from pre iuI confirmed by preg test they re ran and the other two might be one from IUI and blood lot or 2 from IUI. The reason he suggested the possibility of identical twins is because I only had one follicle and it would hVe had to have split. He could barely see those two sacs which is why he thinks those are from the IUI and juat baely 4 weeks. the other one was def from te previous month but I guess they al could have been although I don't think likely. We will see what the second ultrasound shows because it will be a different person so I'm anxious for another opinion. triplets might be an even crazier story!

Obviously no one knows the answer to this "for sure," but I have heard of many situations where a woman has been pregnant with multiples and one has not made it and she was able to carry the other one (or two) until birth. I think the slang term for it is a "vanishing twin." Like I said in an earlier post, I know of two people (one who is a very good friend I have known for about 15 years) who got pregnant with triplets, lost one and were able to carry the other two...all are now healthy babies. I think that usually when it happens this early, the body will reabsorb the tissue.
Another thing I was thinking about (and this is a horrible thing to think of) is the fact that women have selective reductions and still carry the other babies afterwards so obviously either the body rids itself of the baby or babies that are "reduced" or there is some medical way of removing the other baby or babies.
Obviously, I know very little about this, but just wanted to give you comfort that even if the first guy is not a viable pregnancy that does not mean all is lost with respect to the other baby or babies.
