Update: follow-up phone conference (long)

 

We had our ODWU follow up phone call with our doc today.


It went well.


All my test results came back normal. You can see them in my siggy.  My beta ingetrin test was normal as was my antibodies test.


Surprisingly our doc is not concerned with my end of the situation at all.


The real story is that we?re dealing with some pretty intense MFI, which was never wholly figured out/known before.


We?ve needed ICSI with all our IVFs so we knew there was some issue, but not really how much of one.


Using C.CR.M?s specific scale DH?s motility was low at 26% (40% is normal) and his morphology was very low at 2% (4% is normal and 3% or more is where they expect that IUIs would be useful). In addition his fragmentation is quite high.**


When we were there for our ODWU they gave us a list of supplements specific to Mister D?s issues and he?s been taking them since. Our doc suggested that we wait 8 more weeks or so (for a total of 12 weeks), to let those supplements work their magic before running another SCSA test (sperm frag test) to see if it?s improved. She?s assuming that it will.


Assuming the amount of fragmentation improves or stays the same we?ll do an IVF round with C.CR.M with IMSI and PICSI, and then do CCS testing on any decent embryos and an FET at a later date if we are lucky enough to have a normal(s).  I?ll start stims at the end of March, which jives pretty closely with the approx. schedule I thought we?d be on due to how my cycles would fall. Big Smile


If for some reason the fragmentation gets worse I won?t start stims and we?ll need to look at getting testicular sperm from DH (via biopsy) or using DS or taking our journey on another road (like moving to adoption.)


I?ll be on Gonal F this time around, my first time not using Follistim. I don?t have my exact protocol yet but should be getting that soon.


I need to call in with my next period and then we can hopefully align DH?s test and results with when I would be starting my next cycle/stims.


So we both felt good about this approach. I would take any extra wee bits of "healthy sperm" thoughts for us you have wriggling around in your brains in the next 8 weeks.


Mister D and I will need to talk about our options moving forward if things don't look better at the next test. Talking DS is a totally new loop for us that we have not addressed (we always thought it was an egg issue but our doc said there is no indication we'd need DE), we have already broached the topic of adoption... so we'll see.


That's it. Feels good to have a plan and also to have some "down time" before things start rollin'.

** RE: fragmentation - The doc said that all sperm are made healthy and begin un-fragmented but someplace in the process within Mister D they become fragmented. (I thought fragmentation was like an in-born thing, in the creation, but I was totally wrong.) The thing to do to fix it is stop any exposure to harmful things, this includes things like smoking, bad or imbalanced diet, stress, heat etc? And add supplements that help to repair things. We don?t know what kind of harmful things DH?s sperm are encountering. He doesn?t smoke, he is quite a healthy eater, he doesn?t bike or hot tub? but he is quite stressed. Maybe that?s it. Go go gadget supplements!

     I am glad that your follow up went well.  I have been following your journey since my journey is similiar.  First IVF c/p, 2nd IVF m/c 7 weeks.  We are undergoing immunology testing now and starting IVF #3 in Feb/Mar (that's the plan for now...depends on testing).  Anyway....

    I wanted to mention something about the sperm.  My acupuncturist asked if my DH carries his cell phone in his front pocket and if he does he needs to stop doing that as it could affect the sperm.  That might be something to mention to Mister D. 

     Also, if you don't mind could you PM me the supps Mister D is on?

    Thanks.  We did talk about the cell phone pocket thing and DH doesn't carry it in his pocket. Also mentioned was laptop on lap, but DH doesn't do that either!

  • That was really interesting! I'm actually also super impressed that you're doing that for other IFers to benefit from who're reading your post.

    I think the karma points you get from that should be applied directly toward getting you some healthy sperm next time.

    And does that stuff like cell phones and laptops really affect sperm quality? I didn't know that. I always had a terrible suspicion that DH's habit of putting his cell phone directly onto his bits and pieces while he drives was a bad call, but couldn't articulate why.

  • Well that all sounds like great news! I bet it is such a relief to get a plan of action that you are excited about. Since I'm considering ccrm for my next ivf, I am very interested in your reports and testing . I am also curious as to the fragmentation tests they run as my RE doesn't run that test. But, anyway, I'm very happy and excited to see you moving forward!

    Bilateral Tubal Ligation. Stage IV Endo.
    Finally some (tough) answers.

    13R. We have 3 CCS Normal Embryos!


    Lupron Depot: May 2013 to August 2013
    Long awaited FET on 9/3. 2TFR'ed.
    BFP on FRER 5dp5dt: IN SHOCK!
    Beta1 9/10: 152.7! Beta2 9/12: 378! Beta3 9/14: 1224! 

    1st ultrasound 10/1: TWO beautiful heartbeats!  Overjoyed!

    3/22 ER: 25R, 20M, 15F. 9 genetically normal, and 3 survived to Day 5
    3/27 ET: transferred 1 embryo, beta 9dp5dt=163, 12dp5dt=639
    4/25 1st ultrasound at 7 weeks = identical twins with heartbeats?!!!
    PPROM at 31w, delivery at 32 weeks of two beautiful girls

    The main reason I post such long updates, generally, is that I realize there are others (and lurkers) who might take a kernel of what we learned in our journey and be able to apply it to their own situation, or start asking some questions in their own clinic or whatever. That's how I figured out a lot of things myself.


    I'm just catching up since I haven't been on in a while, and I really appreciate the info you provided. You're awesome!

    Right now, my dx guess for us is is slightly fragmented sperm (we've never needed ICSI though, so I'm stumped there) and possible tubal factor (even though my previous lap suggests otherwise), so I'm doing everything I can on my end to correct these "problems". Any suggestions for the supplements? When I asked my RE about fragmented sperm, she suggested I put DH on Fertilaid, which he's been on since December. I've also tried to add C0Q10 to that as well when we remember it.
