We had our ODWU follow up phone call with our doc today.
It went well.
All my test results came back normal. You can see them in my siggy. My beta ingetrin test was normal as was my antibodies test.
Surprisingly our doc is not concerned with my end of the situation at all.
The real story is that we?re dealing with some pretty intense MFI, which was never wholly figured out/known before.
We?ve needed ICSI with all our IVFs so we knew there was
some issue, but not really how much of one.
Using C.CR.M?s specific scale DH?s motility was low at 26% (40% is normal) and his morphology was very low at 2% (4% is normal and 3% or more is where they expect that IUIs would be useful). In addition his fragmentation is quite high.**
When we were there for our ODWU they gave us a list of supplements specific to Mister D?s issues and he?s been taking them since. Our doc suggested that we wait 8 more weeks or so (for a total of 12 weeks), to let those supplements work their magic before running another SCSA test (sperm frag test) to see if it?s improved. She?s assuming that it will.
Assuming the amount of fragmentation improves or stays the
same we?ll do an IVF round with C.CR.M with IMSI and PICSI, and then do CCS testing on any decent embryos and an FET at a later date if we are lucky enough to have a normal(s). I?ll start
stims at the end of March, which jives pretty closely with the approx. schedule
I thought we?d be on due to how my cycles would fall.
If for some reason the fragmentation gets worse I won?t start stims and we?ll need to look at getting testicular sperm from DH (via biopsy) or using DS or taking our journey on another road (like moving to adoption.)
I?ll be on Gonal F this time around, my first time not using Follistim. I don?t have my exact protocol yet but should be getting that soon.
I need to call in with my next period and then we can hopefully align DH?s test and results with when I would be starting my next cycle/stims.
So we both felt good about this approach. I would take any extra wee bits of "healthy sperm" thoughts for us you have wriggling around in your brains in the next 8 weeks.
Mister D and I will need to talk about our options moving forward if things don't look better at the next test. Talking DS is a totally new loop for us that we have not addressed (we always thought it was an egg issue but our doc said there is no indication we'd need DE), we have already broached the topic of adoption... so we'll see.
That's it. Feels good to have a plan and also to have some "down time" before things start rollin'.

** RE: fragmentation - The doc said that all sperm are made healthy and
begin un-fragmented but someplace in the process within Mister D they become fragmented. (I thought
fragmentation was like an in-born thing, in the creation, but I was totally
wrong.) The thing to do to fix it is stop any exposure to harmful things, this
includes things like smoking, bad or imbalanced diet, stress, heat etc? And add supplements that help to repair things. We don?t
know what kind of harmful things DH?s sperm are encountering. He doesn?t smoke,
he is quite a healthy eater, he doesn?t bike or hot tub? but he is quite
stressed. Maybe that?s it. Go go gadget supplements!
