UPDATE how much bleeding is er worthy?

we already know I have a SCH causing bleeding, i was told to expect more spotting, brown maybe even some pink while it heals she could see on the ultrasound that it was not currently bleeding at the time, but im bleeding a lot and its as red as if someone cut me. I filled an infinity overnight pad in about 45 min I don't want to spend my evening in the ER for no reason, but i don't want to do nothing If I need to do something. I just dont know where the line is whats the rule for too much?

ETA last night the on call said no need for er i called this morning during regular hours because i wanted to be sure my doctor knew the bleeding had started again. No doctors in the office until Monday the on call nurse told me to go to the er. So w are in the er waiting to be seen :

  • If you filled a pad in 45 min I'd say it is time to call the doctor!!

  • Rule of thumb is that if you can fill a pad in an hour then it's time to see the doc.

  • I would call your OB. If you are bleeding enough to fill a pad in 45 minutes. That seems like a lot! Wouldn't hurt to call them.

  • SCHs will go in cycles as they bleed out, so the bright red blood can be normal.  That being said, I always called my OB after a large bleed to see what they wanted me to do.  I only went to the ER after the first bleed & huge clots...most other bleeds were large gushes that tapered off pretty quick (back to light flow/spotting) with rest so I waited until the morning and went in to the RE or OB.

  • I've always heard that if you're filling a pad in an hour or less then it's time to call. At the very least, is there a 24-hour nursing line you could call? They could let you know whether to head in to the ER.

I'm sorry you're going through more of this.

  • Ditto.  Word for word what was said at my OB orientation appointment.  One pad in one hour=off to the doctor.

  • I've never dealt with this so pardon my ignorance, but is there anything the ER can actually do? I would think if you aren't hemorrhaging there's nothing they are going to be able to do tonight that your DR's office can't do in the morning?

As I said I've never dealt with this so I could be missing something.

    Due with #5 April 22, 2015. It's a girl!!!!!

Yes it was planned, yes we know what causes that, no we are not on public assistance, and yes we will be getting cable after this. ;)

  • During my miscarriage I was told to go to the ER if I filled a pad in an hour. Honestly, I'd just go in. You could call and ask to speak to an on call doctor if you'd prefer, though.
  • I would call right away. Hopefully the doc will give you advice, but if not, I'd go in.

  • Err on the side of caution. At the very least call either a 24 hour nursing line or an on-call OB.

what would they do? Called after hours line and my doc said no since we know what's causing it unless it continues at that rate i think it's slowing

In the case of a SCH, nothing.  Sleep with your feet up tonight, that always seemed to help me a little

  • Hope it slows up for you tonight. I can't imagine... that has to be just gut-wrenching for you ladies who have had to go through this.
  • With my SCH I had my last gush of bright red blood on Christmas Day and I filled a pad in an hour. I made the decision not to go to the ER because the doctor let me know it could happen, and there is nothing the ER could really do. You did the right thing by calling and letting them know. Let them make the decision as to when to see you next. My doctor told me that since I stopped bleeding later that day that there was no reason to go in just yet.

I've also found that if I get dehydrated it tends to start my spotting again, so keep your feet up and drink lots of water. Thoughts and prayers to you!

Yeah, with a SCH you should be adding about 20 additional oz to your daily intake.  I tried to aim for between 80 and 90 oz a day.

I have had hemorrhage issues in the past and can practically guarantee that they will tell you to go to the ER. I would get checked out for reassurance.

They will likely do a pelvic exam, order a transvaginal u/s to see where the blood us coming from and monitor the bleeding. They will also draw blood to check counts mostly looking at hemoglobin. I don't think there are many drugs they can give during pregnancy but when I had bleeding problems outside of pregnancy, they also have me meds. If the bleeding continues to be heavy or they are concerned, they will admit you to the hospital. It is not something to mess with.
