A couple of questions for any carseat experts. I swear there was a carseat inspector on here at some point!
1) Do you think this product would be safe? I've always been very careful not to add any after market products to the carseat, but even with the convertible carseat at the proper tilt, my DS's head still falls forward. This one looked like it might actually help that. I rolled up receiving blankets and tucked them on the side when he was little, but they don't really help.
2) DS is still rear facing and I intend to keep him that way for awhile. However, due to the impending addition of a second carseat to the car, I was wondering if I can put his seat upright, even though it's rear-facing. (Which will kind of add to the above issue if I do that...) Right now, it's still at the tilt that is demanded in the manual. Does the seat have to be leaned back if it's rear facing; is it a safety issue about how the carseat actually works or is just because of airway issues in little babies?

As far as I've been advised by health & safety, children under a year have to be at a 45 degree angle, children over a year can have it adjusted but must never be more upright than a 30 degree angle while RF'ing.
Aftermarket products not made by the manufacturer of the seat for that specific seat are generally a bad idea... it can compromise the conditions under which the seats have been tested and shown effective. There's a chance that products like that could make them safer, but there's just as good a chance that those products would make them less safe.

1. I can't see the details of that pillow. Does it attach to the harness or is it just free floating? If it's just a pillow going around your kid's head I would see no issue with it (no different than them using their teddy bear or blankie as a pillow, which my kids have done). If it attaches to the harness at all then it's considered an aftermarket product and not safe.
2. Once a child can sit up independently you don't need a 45* angle anymore. How far upright you can install RF depends on the manufacturer. Britax allows either 30* or 35* (I can't remember which). Diono does not have a minimum angle, as upright as you can get it is fine with them. I don't know the rules for the others, but it may be in your manual; if not you can always call and ask, but yes you can absolutely install more upright, which will make it far more comfortable for the person in front of the seat and, based on my kids anyway, likely make the kid happier!
