Update on my little guy

We knew 2 weeks ago that Baby Boy 2 was measuring small (2%ile) while his ID brother was at the 25th%. No issues with Dopplers or fluid. At our checkup this week, Boy 2 dropped to the 1%ile and didn't meet the 100g/week growth they like to see. His fluid had also dropped to the "low normal" range, while his brother's remained stable. That combo got us admitted for a few days for close monitoring.

So far, Dopplers and fluid remain stable and there is no sign of TTTS. They are calling it SIUGR due to uneven placental sharing, and as long as Boy 2 keeps growing and his bloodflows/ fluid remain normal, we can keep going. He'll be small, but hopefully not sick. We got a round of steroids just in case things take a turn for the worse, and go for bi-weekly monitoring from here on out. They released me today.

Have any of you dealt with this diagnosis? Can you tell me about your outcome? I know there is no crystal ball for us, but I'd love to hear any personal stories.

PS: I feel for those of you stuck in the hospital!!!! I was out after 48 hours and was already going stircrazy. Stay strong Mamas!!

My coworker's wife had ID twin boys, and one was always WAY smaller than the other. Fluid levels were always fine, cord flow was fine, etc, but he was just SO much smaller. Their MFM even said she thought something would be wrong with him when he was born, even though the other baby was always fine, so it was unlikely to be anything genetic.

In the end, he was quite a bit smaller than his brother at birth, but perfectly healthy. Just a peanut.

Hopefully this is the case with your little guy too.

Your doing good! Sending T&Ps your way for Boy 2. I hope he can do some growing. I am glad that you got the steriod shots. Stay strong for your trio girl! It sounds like you are in good hands.

My identicals had something similar but they called it TTTS because of the fluid levels, however because it stayed stable my MFM wasn't sure if it was also unequal placenta sharing as well. Baby A started measuring behind at 22 weeks, at 24 weeks his cord flow was absent/reversed which bought me a ticket to antepartum and steroid shots where I stayed until they were delivered at 28 weeks. At first it was seemingly hopeless but we were able to get a few more weeks then what was first thought. He measured 7th percentile and was in total reverse end diastolic flow when they decided to take them out. He came out a little bigger then they estimated but he had asymmetric IUGR meaning his head was normal size but the rest of him was tiny.

They are both doing very well now and thriving. There is still a 2 lb weight and height difference though. Being in the hospital does stink but it's definitely the best place to be right now. Things can change quickly. Getting to 28 weeks is a great milestone too. Sending lots of good thoughts your way and let me know if you have questions.

Added: Generally gestational age is more important than size in terms of survival and outcomes. Many times IUGR babies do very well once they are born because they are under stress in utero. However as my MFM explained, there are increased risks keeping a severe IUGR baby inside, it's difficult a balancing act.

Sendings lots of thoughts and prayers. M has always been more petite than N, and we were a little worried at one ultrasound at 30 weeks because M was a full pound smaller. At the next ultrasound a week later, she was within 3 oz! Then at birth at 35w6d, N still outweighed N by a pound and 3 oz. M is totally fine, just a petite flower

I'm wishing you the best! We've been dealing with IUGR in one of our twins, but fortunately things have remained stable thus far. Our baby girl is in the 1% while baby boy is 52%.

However just curiously - my mfm will only do growth scans every two weeks because he said sometimes there are mistakes on the weekly growth scans (it can show too little growth when in actuality over a 2 week period things are a little more stable). I just figured i'd throw it out there since you mentioned 100g/week. i don't know if i'm the only one, but all of the doctors i've seen in boston refuse to measure on a weekly basis. it's BPP's every time and the fetal II growth scans every 2 wks.

This is what we are doing: twice-weekly doppler/ fluid checks, and every-other-week growth scans. So they're looking for 200g total weight gain at each growth scan. Babies 1&3 had almost exactly that, whereas Baby 2 only grew 150. And he's the small one. Next growth scan will be on the 12th or 13th.

BTW, we are at B&W in Boston- where are you going? I don't think I realized you were a New Englander too

I had a very healthy pregnancy and delivered my b/g twins at 35 wks. I had an u/s everyweek from 6 wks on. They were always very proportionate.

When I delivered at 34.6 wks, my sweet girl was 4 lbs and my little man was 5.9 lbs.

Hudson looked very normal, size wise, to me but, Landry was tiny, tiny, tiny.

I didn't have the opportunity to get the shots prior to them being born. They were in the NICU for 14 days as feeders and growers.

You're at the point where delivery will be okay. Each day you can cook them is fantastic, as you well know. However, you're in the range where it will be okay if you deliver.

Take this time to just laze around. You will not get another opportunity for yourself again for 20 years...you won't. Ask someone to bring you an amazingly soft blanket, fuzzy socks and get into a soap or reality tv show you wouldn't otherwise. After the babies are born it'll never be about you again...always the babies.

We are with Dr. Feinberg in the MFM dept, who is awesome. We live in Central Mass and felt like we'd be in better hands at a place that sees a higher number of triplets, so we make the haul into the city. They have really lived up to their reputation so far- we are very happy there.

I have ID twin girls. One of labeled with IUGR, I think around 30 weeks. Write this stuff down, or you'll forget! They were born at 36w3d. I went into labor the night before my c-section, which had been moved up because the smaller baby was shunting blood to her brain. She was born weighing 1.5 lbs. less than her sister and is still about a lb. lighter and an inch shorter. No NICU time.

They are 10 months old now and both are thriving and healthy .

Best wishes for you and your sweet babies. I remember feeling so guilty that one of my babies wasn't getting as much nutrition in utero as the other and hating that I couldn't do anything about it. I don't know if you're dealing with those feelings or not, but try to let them go and focus on baking those babes as long as possible.

My boys were mo/di. My baby B was always measuring small, but also no signs of TTTS. We were closely monitored. I was induced at 37 weeks because Baby B was continuing to lag in growth. Birth weights were 5lb 9 oz and 4 lb 12 oz. No NICU time, no health issues.
