This is pretty much what happened to me with my c/s, except that baby was OP and did have a pretty good cone head when she came out.

Yep, 2 1/2 hours of pushing with an OP baby and a nice side conehead (head was also tilted to the side and up instead of tucked). No doubt in my mind that had I been in a hospital I'd have ended up with an unnecessary c-section.
