I haven't been on here much in a while because I was home all summer and the beginning of the school year is a bit crazy, but I have a question. Em had an ear infection 2 Thursdays ago. She went on amoxicillan and had major loose poop issues. Because of this she got a really bad diaper rash. She stopped her medicine this Sunday morning. She poop was still loose, but not as bad and her diaper rash seemed to be clearing a bit by Monday when she had her 15 month appt. She got her shots and the doctor said just to continue with the A and D cream and let her have lots of naked time, which we had been doing. Her poop and rash have both gotten so much worse in the last few days!! I called and talked to the nurse yesterday. She said to keep doing what we are doing and just put her in the tub or at daycare wipe her with water on soft paper towels when she poops. Well she poops loose 6-8 times a day and it's hard to get her clean. She screams and squeezes her legs together even when I change a pee diaper. It is so sad and seems so crazy to me that there is nothing else we can do. This is her third time with really horrible diaper rash, but it has never lasted this long. Does anyone have any advice or things that worked for them?

first - are you sure it's not yeast? Yeast is REALLY common post antibiotics. The worst rash Lexi ever had was post-antibiotics, and it went from zero to awful broken skin in one day. You have to treat yeast rash completely differently since it's needs an antifungal type cream. (we got a heavy duty prescription, but I know some pedis recommend lotrimin).
If it really is just from the moisture, we used aquaphor a ton when Lexi had a tummy bug and had diarrhea all day. We'd get her really clean, pat her completely dry, and then coat her really well in a thick layer of aquahpor. It was a pretty good barrier against the next poop, and then we'd start all over. We also did oatmeal baths to soothe her sore bottom. It seemed to help!
I agree with PP, but I would also try a different rash cream. Sometimes the cream itself can burn them or cause other issues. In cases like this I would also use plain old vaseline.
I agree with the others. I would use vaseline instead of a&d.
Also, try to feed her foods that will bind her stool up a bit. Bananas, bread, apples (not juice), fiber foods. Also avoid citrus fruits. Hope she feels better soon.

For rash comfort, a Pediatrician I saw once suggested baking soda in her bath water while she sits there. Or oatmeal bath. The baking soda neutralizes urine acidity. When Liv had a bad rash, I washed her bum with water + baking soda mixture with each diaper change instead of the wipes. And lots of naked time.
The baking soda bath is wonderful!!!! When Seamus had his bout of diarrhea and horrible diaper rash that creams did nothing for, we did this. I filled the tub up to about his waist deep and sprinkled about 1/3 C of baking soda, then sloshed to mix thoroughly. I plopped a few toys in and tried to get him to soak as long as possible. I'd tell him to soak his bum everytime he stood up and he'd sit and say bum. Lol. After about 10 minutes I patted him dry. Then I laid out a big blanket in his room and let him be naked for a long time! I slathered desiten overnight on and put a diaper on. Change often! Like ridiculously often...every hour, two at the max, pat dry with paper towel if she's wet, then let her be naked for awhile if you can after every change. HTH!
