  My second twin was delivered by breech extraction! I know that's a little different since it's easier/safer when he's coming out right after another baby (who was fortunately head-down); my resources are all on breech delivery of a second twin so I'm afraid that is no help to you. It really wasn't bad though. Good luck to you, whatever ends up happening! I agree with pp's that practitioner skill makes all the difference in how safe it is; I switched OBs at 31w because my Baby B kept flipping back and forth between vertex and breech and I wanted the breech extraction option if possible.
