Clomid when you're already ovulating?

I tried to search the board to make sure this hasn't already been discussed - my apologies if it has!

I just FINALLY got in to see a fertility specialist after 2.5 yrs (healthcare is nuts where I live). I've had numerous progesterone tests, a transvaginal ultrasound, an HSG  etc. - nothing showing any kind of physical issue or any answers. Husband has slightly lower motility but that's it. More details to our story, but for brevity's sake...

Annnnyways, first time meeting the specialist and after going through all the history she says she's going to start me on clomid. So I said, "But... I'm already ovulating just fine..." and she said that they just "sometimes do clomid" but really didn't explain why. I honestly don't have the luxury of switching docs (without having to wait 6-9 more months) so I'm going to try it (not until Sept or Oct) but I still don't want to waste a bunch of time, if it's pointless...

I wondered if anyone can explain why I need clomid if I'm ovulating just fine (sometimes better than fine) right now? Any other ideas/options you may have been prescribed that I could ask about instead?

Regularly access alternative medicine to supplement a lack of resources in our geographical area.
  • Three rounds of clomid (50mg days 3-7) were useless.
  • IUI#1 w/clomid & HCG in May 2013, BFN.
  • IUI#2 w/clomid & HCG on July 23rd. BFN.
  • IUI #3 in August cancelled last minute, due to lab error.
  • IVF#1 - Started Gonal F stims October 13 2013 for IUI. Over-responded with too many follicles, so converted to IVF. Retrieval October 29th - got 5 eggs, 4 fertilized. Had 2 perfect little embryos transferred on day 3 (Nov. 1). The remaining 2 didn't survive to freeze. Got our BFN on November 13th 2013.
  • IVF#2 - Started stims August 21st. Triggered with Suprefact, started on dostinex due to risk of OHSS. Retrieval September 5th - got 27 eggs! 21 embryos on day 3. Embryos deteriorated quickly. ESet 5day blast on Sept 10. No embryos high enough quality to freeze. BFN came Sept. 22.

Reserved a semi-known proven donor in August .

Waiting for consult/screening and protocols with estimated fresh cycle timing in October

Re: Clomid when you're already ovulating?

  • It is not uncommon to prescribe Clomid to women who ovulate.  I think it can make a stronger ovulation; it could also make you produce two (or more) rather than one mature follicle which increases your chances that one will fertilize.  I have ovulated every cycle that I charted and/or was monitored (14 cycles total), but was given Clomid to increase my chances of conceiving.  Unfortunately, 5 days of 100mg actually caused me to NOT ovulate, which my doctor said is rare for people who do ovulate.  They put me on another 5 days and I did ovulate, but it did not result in a pregnancy.

    IVF October 2012: 22 eggs retrieved, 17 fertilized, 5 frozen
    ET #1: 1 blast = BFP; Blighted ovum discovered at 7w5d; D&E
    FET #1: 1 blast = BFP; Missed m/c discovered at 9w5d; D&E
    Karyotyping: normal ~ RPL Testing: normal ~ Hysteroscopy: normal
    FET #2: 1 blast transferred 10/25; BFP 10/31!
    EDD 7/13/14 ~ Induced at 37w4d due to pre-eclampsia ~ Born on 6/28/14

  • Clomid can be used to ovulate 2-3 eggs at a time in women who already ovulate. This will increase your chances of conceiving and doing IUI as well may be smart to combat a mild MFI problem. Good luck!

  • My first u/s at my consult showed that I had just ovulated so we know that I was ovulating on my own. I still did clomid and iui together. It can help you have a stronger ovulation rather than one that's there but not great. Think of it like mail. You can still snail mail but email with a confirmation email is faster and easier. I only ever had one follie each cycle in 4 medicated cycles.

    2 large subchorionic hematomas & no heartbeat at 7w6d   
    D&E 8/18/12 Sonohysterography found septum and necrotic tissue.

    IUI #5-7 50mg Clomid + trigger = BFN  
    IUI #8 Femara + Bravelle + HCG + Progesterone = BFP 3/27/13

    D & C 5/10/13  Triploidy 69 (paternal inherited)
    IVF #1 with ICSI and PGS 11R 8M 5F 2 biopsied/frozen
    PGS results = 1 with trisomy 13 & 1 good embryo for FET 
    FET #1 EV, estrace, nitro patches.  Cancelled due to thin lining
    FET #1.2 oral estrace, f'ing nitro patches and no delestrogen.  Transfer 12/31. BFN
    PAIF/SAIF welcome
    Surprise BFP on 6/13/14  Our only unmedicated bfp ever.
    Beta #1 339  Beta #2 649 44 hour doubling time
  • What PPs said. I also ovulate fine on my own, but took clomid in the hopes that 2-3 follies and an IUI would give me much better odds. It is a fairly common thing for REs to do, and you should not be worried about the competence of your RE in suggesting this. :)  
    I had some issues with clomid, so we switched to femara after two cycles (essentially the same treatment with a different drug,) at which point we could have done an inject IUI but chose to go with IVF. You are doing a common first-line treatment...hopefully it does the trick! GL!
