when follies mature...two doctors two opinions...

So I had monitoring this morning and had an 18mm and a 14mm and a few smaller ones. Based on my previous experiences and a long conversation with a different RE I assumed I would be told to trigger for either Tuesday or Wed night and then IUI either Thurs or Fri. Instead this new RE told me to wait for trigger until Saturday and IUI on Monday. She said that with oral meds she thinks it's better to let them mature into the upper 20's. Thoughts? I'm doing OPKs to avoid missing a natural surge, but I'd much prefer trigger for timing since donor sperm live so short. Plus I'm terrified of missing a natural surge even with OPKs.

  • Hmm that's really interesting.  What cycle day are you on and which days did you take the meds?  We always trigger me when there's an 18 but I haven't gotten pregnant yet so who the heck knows.  Keep us updated on your cycle.

  • We usually trigger when there is one in the upper 20s, but same here haven't gotten pregnant yet so not sure who is right!

    After 2 years of IF workups/treatments and 2 IUIs, we have closed the door on fertility treatments.

  • I took Femara CD 2 to 6, today is CD 9. I've always triggered around 20mm, but like you said I'm not pregnant yet.... also in my chemical pregnancy month I think we triggered at more like 23mm. But I specifically asked my normal RE about this and she said it was a coincidence and they're mature at 20. Hm.

  • waiting until this weekend is WAY too long, you might lose that 18mm one and then miss your chance.  i always triggered in the low 20's, even with clomid

  • My RE seems to be content with 16's and above...but he bases it off of the bloodwork too. My first cycle I had a 16 and he had me trigger the following night after my follie check so it still had 3 days to further mature. This last time I had a 16 and a 17.5 and he had me trigger that same night so they had 2 days to further mature. I assumed the size was only part of how they make their decision and the bloodwork was the main decision maker. But I'm still pretty new to this so I could be way off!

    First Ultrasound at 5w1d showed a miracle UTE baby! And right ovary ovulation to left fallopian tube.

    JD arrived at 38 weeks on October 20, 2015.

  • Hhhmm, I had monitoring this morning and after seeing blood test results and 3 follicles (17, 16 and 12), the Dr is having me trigger tonight and IUIs tomorrow and Wednesday morning. I have always read and been told follicle size should be at least 16 for trigger/IUI.
    Me: 32, DH: 34.
    Trying since Jan 2011. Unexplained IF.
    2 IUIs = BFN.
    1 IVF (Dec 2013) = BFN.
    FET, 2 frosties (June 13, 2014)

    14dp5dt-June 27 -BFP, beta 2061. 2nd beta >5000, 3rd beta >5000, 2 sacs 06/30.
    Twin Girls - 02/11/15 - at 37 weeks (no NICU, home with me at 3 days).
  • My RE doesn't believe in midcycle blood work on oral med cycles...good for the price of things, but wishing right now that I had an e2 number. I know it's pretty universally accepted that follies can be triggered smaller in a injects cycle....but the whole thing is throwing me for a loop.

  • I am not doing IUI- just intercourse. But I had my monitoring appt today as well i had a 14 and a 15 and a 7.  I am triggering on Wednesday night and are to have sex thursday and friday. Guess every RE is different..but that seems too long. If you arent ok with it call him.
    Me: 24 DH: 27 12/7/2011- Faint BFP #1- 12/18/2011- early m/c at 5 weeks- CP 1/30/2012- Faint BFP #2- 2/17/2012-early miscarriage at 6 weeks- CP RLP done- 1 copy C77T hetero MTHFR-Slight hypothyroid-normal HSG. 8/2012 and 9/2012 Clomid + TI= BFN 10/2012- follistim 7.5 and IUI #1- BFN (pressured into this IUI) 12/2012-BFP #3- 2 days later started bleeding. 3rd CP. 1/18/2013 BFP #4. Beta #1- 14.7(10dpo) 99.5(13dpo) 263 15(dpo).STICK LITTLE PEANUT- WE LOVE YOU!

    what?? e2 levels can tell you a lot , and i would want everything in line with NO guesswork when putting myself through medicated cycles. why  wouldnt they do a simple blood test??

    find a new RE. really.

    Yeah, I know E2 levels can tell you a lot, but I don't think it's medically irresponsible.  In any case, this RE accepts my insurance and is in a location I can stomach.  And if this cycle fails we're doing IVF next anyways, where we'll get the full set of monitoring.

    But thank you to everyone for your responses.  I'll post if there's any update.  I did a bunch of reading on the internet and did find several references to ovulating with oral meds with follies in the mid to upper 20's...  but most people never get there since many REs have you trigger at 18 or 20.  I keep getting stuck on the fact that I had a chemical pregnancy the one cycle we triggered a bit later, and of course the fact that I'm not pregnant after so many triggers at smaller sizes.  I'm doing like 4 OPKs a day for now.
