My son first showed signs of thinking about climbing out a few months ago so we started thinking about it, but weren't going to do anything until he moved to his new room in a couple months or actually climbed out. However, last week he attempted to escape and ended up hanging upside down out of his crib, holding onto the slats. I saw it on the monitor and somehow managed to run upstairs and catch him before he fell, but he moved to the toddler bed that night. The transition went great and we haven't had any problems at all.
It took 3 1/2 long years, but we finally got our little miracle!

Our pediatrician told us it was 3 years old, 35 inches tall, or when they started climbing out, whichever comes first.
We transitioned Liam around 20 months because he rejected his crib after sleeping on a futon mattress at the in-laws over Christmas.
I'm not sure why the pedi would mention a height. A toddler mattress is the same as a crib mattress. Kids climb out at all different heights.
Personally I'd like to keep DD2 in her crib until close to 3 years unless she climbs out first. DD1 climbed out at 22 months, and bedtime was rough for a long time after that. We also skipped the toddler bed and went straight to a twin bed with rails. I plan to do the same with DD2.

We just did it tonight, were the crib turns into a day bed... and its tough lol i feel like my baby is grown and i am sad about it... and holllyyy crap trying to keep her butt in there is tough. she is 16 months old and was climbing out onto her changing table. scary. good luck ladies

We are thinking soon so we can have the crib for the baby. DD is almost 35 inches so if that's the guideline then we wouldn't be able to keep her in there much longer anyway...

Our pedi said to transition when he starts to climb out or when we start potty training. He hasn't even attempted to climb out, but he sleeps in a Halo sleepsack, which makes climbing out a lot harder. And we definitely aren't ready to potty train yet! He sleeps great in the crib, so I am keeping him in it as long as possible.

They make "big kid" sleep sacks up to like 5T.
