I wear them alone and they're great. Personally, I don't miss the underwire of my regular bras. Lately I've been layering them with sweaters. I like the ones from Target the best.
They provide me with enough support, although I'm not particularly big (B cup prior to pregnancy and probably a C cup now). Like PP said, I like Target's nursing tanks best.

Like PPs, I feel comfortable wearing them without a bra ... at home, anyway. It doesn't look totally appropriate necessarily, but it's just me, the baby and DH so who cares? When I go out, I wear a nursing bra underneath or a sweatshirt (or the baby in his wrap) on top. I'm somewhere around a 38ish D maybe, if that helps at all. And I agree - Target has the only nursing tanks I have liked at all so far.
I am currently a DD and I find them supportive enough to wear alone. I have the Target ones and I really like them. I am wearing them under sweaters right now. It's nice to have the extra layer so when you lift up your shirt to nurse you don't a) freeze and b) expose your belly.
It all depends on the style of nursing tank and your bra size. I bought one from Motherhood Maternity and it is not supportive enough to be worn alone outside of the house. I actually don't even really like it for around the house.

Not nursing yet, but I leak a lot and have started wearing them to help with clean-up. I'm also a B cup pre-pregnancy and about C right now. My nipples poke through a lot and I would never wear them alone in front of company/public. Layered with an additional bra for outside is fine.

Re: Stupid question about nursing tanks...
I noticed a huge difference in the "perkiness" of my breasts with my first daughter
and I am a 38E when I breastfeed, 38D when I am not.
Now I wear a nursing bra 24/7, even when I wear the nursing tanks. I was one who used to HATE wearing bras and still do. It sucks.
I have pretty sizeable girls and I wear mine alone. If you get the right size you should be fine with just a tank.
ETA: By "alone" I mean without a bra...I always wear a cardigan, shrug, or something on top.
