TTC After a Loss

**CCH** and others - my first RE appt for multiple losses

Quick background: after my second loss my OB decided to go ahead and diagnose me with multiple losses and run a RPL panel on me. We did chromosome studies on the baby which revealed Trisomy 21 as the cause for this loss, but the RPL blood work was done before we found this out. My RPL came back fine except one slightly elevated antibody. I had asked my OB about some additional tests (like MTHFR) but he felt it was outside of his scope to test for, so he offered to refer me to a RE.

The appointment started out in the doctor's office where he reviewed my stack of medical records from the OB. He basically said that there's no reason to do a S/A on DH at this point because he's gotten me pregnant twice, and there's no reason to do a HSG because I've had two intrauterine pregnancies so he isn't worried about my tubes. He then took me into an exam room and did a quick physical and a transvag u/s (glad I shaved my legs yesterday). He looked at my ovaries and said they look "consistent with my age" meaning no evidence of DOR, but he did notice the lining of my uterus looked a little irregular, so I'm having a SHG done next week to look for scar tissue from my D&Cs or other abnormalities.

Aside from that, he basically said he plans to "stay out of our way" for the next 6-8 months. He said to keep using OPKs, but to quit temping because "it's not that accurate" and might make me crazy (haven't decided if I'm going to stop or not). He said if we're not pregnant in 6-8 months we'll reevaluate since that would probably change our issue from recurrent losses to some sort of infertility issue. He also said if we ended up with another pregnancy with chromosome abnormalities, something like IVF with PGD might be more appropriate since my risk is greater than others my age. At this point, though, he said our prognosis for a healthy pregnancy was "excellent".

So, I feel pretty good about it, and I'm glad I went. He didn't try to push us into fertility treatments (that aren't covered by insurance), and only ordered things he felt were necessary based on my history and current situation. The girl who scheduled my SHG also said she reviewed my insurance benefits and said since I have coverage for diagnostic procedures, my SHG should be covered.

I hope this helps you make your decision about what to do next, CCH, and anyone else trying to decide if a RE referral does anything for recurrent losses.

  • Sounds like good news for you!!! I like that he didn't push you in to treatment! 

     

    Im hoping for a bit more intervention, in the way of sustaining pregnancy. I've ready got an unexplained IF dx under my belt and we've had all basic testing + RPL done. Guess I will have to wait and see!!

  • I'd push for it and see what they say, for sure. I was happy he didn't push treatment either -- that was DH's biggest reservation about going this route.

    I hope you get a good plan set up. Hopefully my experience shows that at least you can go to a RE and not walk out with drugs or major fertility treatments.

    yeah i'm definitely glad to see that not all RE's jump to treatment first. the RE i saw in may wanted us to go right to iui/ivf, and we hadn't even been back to TTC for a year yet so it just didn't seem like that was the best idea yet. i was frustrated that she wouldn't consider some other options before moving on (clomid, injectables, somethng, anything). i really felt like she was pushing for her own profit gain, so i've been afraid to see another RE partially for that reason.

    here's hoping you don't need to consult with the RE about an updated treatment plan in 6-8 months!

  • Thanks for posting! This will be my first cycle TTC after 2 losses and I have been going back and forth on if I want to do any testing or just go for it and this has given me more to think about. I feel the same as you, I would like answers but don't think I want to go down the infertility treatment route.

    I'm glad you got some answers and wish you best of luck getting your take home baby!
