Success after IF

On or off the BCP?

I currently have an RE but going for a second opinion because I am not happy with my follicle results and want to make sure that this Dr. is doing everything they can. Had a second consult with another Dr.who mentioned not doing the BCP because when I do take them they might be suppressing my ovaries? I produce about 7/8 follicles but felt I should produce more. My FSH is 6 and age is 40 - no other issues except DH morph and motility issue. Oh, I was on 450 Gonal f and 75 Menopaur and Ginirelex last time.

  • I have a very "active" ovary so BCP is essential to calm them down a bit before stimming. However, I know that there are certainly cases where BCP can oversupress someone and an RE wouldn't recommend them. I think if you are unhappy withj current RE and feel more comfortable with the change in plans of the new on, there is certainly no harm in switching.

  • there is an element to the BCP prior to IVF that actually "helps" the ovaries... I want to say the testosterone effect from them??? So I was told that I had to take them, at least to just downregulate things for a few. (I shouldn't be on them d/t blood issues. And I hate them. lol)

    Two things, though... at age 40, that's kinda what I would expect your oves to produce. It doesn't surprise me THAT much. (I say that b/c I'm old too)

    However, I will say, that my best stim out of 3 fresh IVF's was the one where we did VERY minimal lupron and BCP... I want to say just shy of 2 weeks? And I had a few more retrieved, and 100% fert rate. So I'm a fan of "just a nip" prior to stimming.  (FWIW, I was on 450 follistim, 150 meno and lupron to hold me back. This was "hard and fast" compared to what I usually did, pretty much shoving the "low and simmer" theory out the window for me!)

  • I have done 3 IVFs with 3 different protocols and have always used BC. 

    IVF #1:  Lupron 1 week prior to stims and 1 week into stims (Follistim & Menopur).  Result:  4 eggs retrieved, 2 embryos remaining at day 3, and a BFP.

    IVF #2:  Bravelle & Menopur with 5 doses of Ganerelix.  4 eggs retrieved, 2wo embryos remaining at day 3, and a BFN.

    IVF #3:  Microdose lupron (10 units) once a day and Bravelle & Menopur.  9 eggs retrieved, 7 blasts remaining at day 5, and a BFP.

    I was 35 for #1 and was 37 for  #2 and #3.  I have PCOS.  For me microdose lupron was the key to my most successful cycle. 

    ETA:  I know I didn't answer your question, but I guess my main point is that there are other protocol changes other than BCP that might be a factor, and that quantity isn't everything!
  • Yes, you were helpful . I have two other second opinions to get and waiting for the other one to get back to me because some of my records from my second IVF were missing.
