Before TTC checkup - OB/GYN, Family Dr., or CNM?

Hi all, I need advice on who to see for my pre-TTC checkup.  I am planning on using a birth center for delivery (if ins will cover, etc.).  However, I think I would still like to at least be in contact with an OB/GYN just to get a different perspective or if I need to deliver in a hospital.  Also, we were planning on TTC next summer but, well, that just seems like too long :) so we are thinking of trying as soon as I go off the pill and it is safe, etc.  So who is the best person to make the initial dr.'s appt with to begin answering all of my questions, blood work, etc.? My last gyno exam was with my family dr., so I don't have a regular OB/GYN.

  • You don't need pre-TTC bloodwork, for one.

    For two, get a recommendation from the birth center which midwives they'd suggest if you want to go for a pre-TTC checkup/Q&A. The midwives that you'd meet will more than likely have contact with their own set of backup OB's that you would meet during a pregnancy.

  • I would make the appointment with whoever you want to handle your prenatal care.  If that is a CNM, then make it with her. 

    If you are being seen by a CNM, why do you think you need an OB/GYN to judge whether or not you can deliver at the birth center? Just to warn you, a lot of OB/GYNs will recommend you deliver at the hospital for no reason, other than they don't see birth centers as good places to give birth or they would rather have you pay them to deliver your baby, versus have you pay a CNM.

    you don't need such an appt unless you have a pre-existing health condition.

    just go off the pill and take PNVs.

  • I didn't have a pre-ttc check up. I just asked some questions at my yearly appt. You wouldn't have any blood work done.

  • I didn't have a "pre" TTC check up nor do I see the point in it really. What they are going to tell you is chart to know when you ovulate, have lots of sex and depending on your age (I am under 30) it could take up to a year to conceive.

    I don't have anything on who to choose. I just went with my ob/gyn b/c I like her and feel comfortable going to her. I agree though that most ob/gyns will say to deliver in a hospital.

  • I informed my doctor at my yearly check up that we wanted to TTC this summer. She actually ran a full blood panel on me, to see if I was immune to chicken pox, measles, etc.

    So, OP, if you haven't had chicken pox, you may want to have blood work done to see if you are immune or get the vaccine.

    I usually don't advocate such appointment, but this is a really good suggestion if you are hoping to use a birth center.

    I agree - run a thyroid panel as well if you've never had that done - check your TSH levels in case you are undiagnosed hypo or hyperactive thyroid - from what my endocrinologist told me, a lot of women are.

  • The full blood panel is what I was thinking of.  I also want to know if I am susceptible to toxoplasmosis (sp?) so I will know whether to be concerned with gardening.  I also wanted an opinion on how long to (if at all) I should wait to TTC after going off the pill and if I have been taking prenatals for long enough.  I just found out the birth center is an out of network provider for my insurance, so I guess I will make an appt with a covered OB/GYN just for an information meeting.

    wait.... We have to have sex???? OP, I had my thyroid checked, but that was a concern anyways.

    I must say, Aardvark, I was shocked.  Shocked, I tell you. 

    Well darn, if I had known that, maybe it wouldn't have taken me 8 whole months to get KU!

  • You don't need to wait a certain amount of time after going off the pill. The only issue there is being able to calculate when you conceive, so if it makes you feel better, wait until your natural cycle regulates itself so that you know when you ovulated.

    I got pregnant only a few months after going off the pill. I wasn't on a 28 day cycle yet. I kept track of the days we had sex and was able to narrow it down by what I thought was implantation bleeding. I ended up getting an early ultrasound and they are pretty accurate for determining due dates. So that's where I got my "real" due date.

    Personally, I wouldn't bother having a MW and OBGYN unless they worked for the same practice or hospital. Just go with who you're comfortable with and ask them to run a blood panel if you think you need it. If it's not something they typically do, and I think most OBGs and MWs don't, they can refer someone.
