LC: Oversupply/Engorgement Issues (Long)

Hello Deirdre,

My DD is just over 6 weeks old. She was born tongue-tied so we had latching issues once my milk came in, and ended up introducing bottles of pumped milk early because we couldn't feed her. We fixed her tongue-tie on Day 4, but she continued to have problems latching on one of my breasts due to a flat nipple as well as continued latching incorrectly (clamping down, etc). We met with many LCs and were told to keep working on the latch, but in the meantime, I needed to be pumping every 2 hours to help establish my supply since we were having problems with LO doing that in the early days at the breast. She was also having problems going back to the breast, but we were able to get her back to breastfeeding and not just on bottles. I was also told that until I could get her on the right, I needed to pump that breast empty each feeding to help maintain supply. We were able to do some exercises with her and she started using her tongue correctly with breastfeeding - so that improved. DD has become a one-side feeder due to always starting and finishing a meal on the left.

So, fast forward to these past 2 weeks - once DD got a bit bigger, and with the initial help of a nipple shield, I was able to get her to latch on the right (without the nipple shield). She now will feed on both the right and the left breasts, but sometimes gets frustrated with the right due to the slower letdown.  She also still takes bottles of pumped milk.

Now, I'm having oversupply/engorgement issues, due to the pumping (even though I stopped pumping every 2 hours, I still have to pump one or the other of the breasts at each feeding, due to engorgement issues). I spoke to a midwife yesterday, and she agreed something needed to be done and suggested that I only "pump for comfort" and bring the baby to breast as often as possible. I've been working on that, as well as trying to switch her to each breast during a feeding (based on midwife's suggestion), but when I worked on "pumping only for comfort" - I ended up waking up even more engorged than ever before, with hard areas in my breasts and I believe clogged ducts (felt like peas under several areas). Even when DD feeds off one breast, she is still not able to soften the entire breast, due to the oversupply. I'm concerned about getting an infection by leaving these hard areas after each feeding.  DD feeds every 1.5-3 hours.  

Any suggestions to help regulate my supply? I will be returning to work in a few weeks, so I know I will need to bring my supply down, but also have enough to keep building up my stash since I'll be away from DD for 10+ hours of the day. Thank you!
