no help on #1 but we never did a pre-conception visit. it's a waste IMO , they're going to tell you to take prenatal vitamins and maybe cut down on caffeine if you drink a lot. that's about it.
My husband are trying to get pregnant right now with IVF. I have already chosen who I would use ( I have an extensive health history due to cancer tx at 23). I actually also work with a lot of these doctors everyday at BWH as I work in the cord blood donation program. I would be put in a higher risk category right away if we are lucky enough to get pregnant and would trust Brigham to deliver my child. I very much like the Katz, Cline, Zinner group and would probably pick Dr. Zinner or Dr. Katz.
I've seen many of the midwifes and Dr's in action because we go into the c-sections/ and vaginal births to get the placenta for the donation. Both these doctors are amazing!! Both are very professional but so sweet with their patients! I'm currently being seen in the ART clinic and they had asked who I would use and I told them to which they replied "very good choice"! Two of my colleagues (one is a nurse the other is my co-worker) are currently pregnant and seeing Dr. Cline... To me it says a lot about a doctor when others in the medical profession who work directly with them use them for their own children!
Good Luck with everything!!

My friend works at One Brookline. Very good and popular office!
I delivered at B&W and went to Harvard Vanguard in Quincy (they all deliver at B&W). I loved it (was more convenient for me that One Brookline). They have midwife program, offer pre-natal yoga classes and almost all tests can be done in the building. Your appts. are split between your OB and a midwife (if u choose to see a midwife).
I did not have a pre conception appt. I had told my P.C. at my previous annual visit that we would start trying. She basically said what the other poster said... take prenatal vitamins! Oh and keeping good track of my cycles and perhaps use an ovulation test to know when I was ovulating. Those never worked for me but thought I would pass it along.
I've been going to the Harvard Vanguard Kenmore office and have been followed by Ashley Wilson (certified nurse midwife) and Dr. Dowling (she's the head of the Dept). My husband and I love Ashley. I've met two other patients of hers and they speak highly of her as well. I really hope that she is on duty when I deliver.
Dr. Dowling is very nice but we've only met with her once since we like Ashley so much. Ashley gives us a lot of time in our appointments and answers questions confidently. She's been really good at explaining tests and our options, reassuring us as first time parents, etc.
I didn't specifically have a pre-conception appointment but as pp, I mentioned it during my last yearly check-up that we were going to start trying. Ashley gave me good info on ovulation and general health. I thought I was informed but she did teach me a few things.
