Gonal-f is a brand of FSH. Follistim is another brand but they are the same thing - pure FSH. It's a drug that stimulates your ovaries to produce follicles. In an IUI cycle, they adjust your dosage so you don't produce too many follicles, unlike IVF where you try to produce as many as possible.
Usually sold in units of 300, 600 and 900 pens or vials. You can look up pricing on Freedom Pharmacy's website. On my IUI cycle I used about 1350 total units.

My nurse donated me a sample pen of Gonal-F to use for my last IUI but she estimated it to be between $300-$400 for a 300ml pen OOP. I would look into the website that provides fertility meds, I *think* it may be more cost effective that way.
I think how much you take varies per each person and their Dx. I took a total of 3 doses, each 75iu. I know my dosage was lower than a lot of others.

I started it on day 3 of my last cycle, and took 112.5 units a day for 7 days, which was about one 900 unit pen. There are two approaches to dosing: start low and increase as needed, or start higher (usually 150 units/day) and decrease as needed. We did the "high" protocol, but started at a lower dose because I responded really well to Clomid. We didn't have success with 3 cycles of Clomid, so moved to injectables for IUI #4. There are better success rates with injectables, they are just a lot more expensive and you need more monitoring.
I had an ultrasound on d7 with 7 "lead" follies, didn't change the dose, and had another ultrasound on d10. We had 2 follies >17 mm and 2 >15 mm, so triggered that day for the IUI.
As for cost, if you don't have insurance, I think freedom fertility is the cheapest place to get it. They do have some discount plans through the pharmaceutical company that you could look into (my browser won't let me link to it, but it's here: https://www.fertilitylifelines.com/payingfortreatment/index.jsp). Also, it's worth calling around to other pharmacies. I go through the university here and they get a deal on medications through the state, so I only paid $80 for a 900 unit pen, but that's much much lower than you'll find most places.
