I'm confused at lost at what to do next. DD was born at 32 weeks 1 day in Dec, and was only in the NICU for 20 days. She tolerated all of her bottle feeding really well. Never took to EBF, so I continue to EP.
Over the past 3 or 4 weeks she cries for at least one hour when I start her bottle, will cry during, and then by the time she eats 2 or 3 oz, she is so tired she just sleeps and ignores the last 1 or 2 oz. The pedi switched us from Neosure to Enfamil supplements. DD acts like this with all BM, all formula, or a mix of the two. Pedi suggested all BM for a week and to see DD improved.
Since 4/5, she has lost 1lb 4oz. YIKES. She is down to 8lbs 2oz now, she will be 4months actual tomorrow. Could this be a milk protein allergy? The first appt they have is Tues at 11. Help! Any advice, tips, comments?!

I don't know much about milk protein allergies, but it sounds like reflux to me. Especially since she is crying during the feeds. She could possibly be having silent reflux, where she's not spitting up, but is still refluxing up her throat and is in pain.
There are several medicines you could try - my son is on Previcid and it makes a big, big difference.
I would agree with PP that this sounds like reflux. My girls (also born @ 32 and 1!) had pretty severe cases and they would cry a lot while we were feeding them. They tried Zantac, and are now on Losec to treat, and it seems to help a lot (also they are quite a bit older now, so that's probably helped too).
I know lots of people advise to keep your baby quite upright while feeding, and for 30-min to 1 hr after, although we never found that helped too much. I fortify my BM with formula to increase calories because weight gain has been an issue for them. There is also some belief that cutting certain foods (caffeine, dairy, etc) out of your diet, can help improve reflux in breastfed babies...

I agree, it sounds like reflux to me too. No kiddo when they feel poorly eats well. I'd ask about trying a reflux medication.
Food intolerances can definitely make reflux worse, so trying to modify their diet is not a bad idea either. I would just change one thing at a time though or you'll be left scratching your head as to what to blame if things get worse, and what to credit if they get better.
sleeping at an incline definitely can help.We had Evan's entire matress up to 45 degree angle and had a sling we strapped him to the bed with. Stopping to burp frequently, and holding upright for 20min after eating can also help.

My guy has a milk protein allergy AND reflux. We didn't find out about the allergy until his poop was green with blood because we thought the reflux was the only problem.
Sounds like reflux to me. We have a rock and play, but found that he likes to sleep in the Nap Nanny much better. Between the angled sleep and a dose of pepcid 1x a day he is very comfortable now!
