We are transitioning our DS from the swing to the crib and he has been doing pretty well. We bought a crib wedge to keep him at a slight incline still.
Last night though I woke to him screaming because he had somehow gotten a bit sideways and rolled down the wedge.
How are you preventing roll-aways from happening?

Have you considered putting something under the mattress instead? These crib wedges and positioners have been recalled and/or not recommended anymore due to these slipping issues you're having.
Babies can slip or get wedged and can suffocate.
I'm not sure about the PPs, but we have a pillow under our mattress to elevate it and I think a lot of the wedges also go under the mattress, but the baby still moves and slides down if not held in place.
