The one time I called with TCPA it's been a nurse, but I'm quite certain she wasn't a nurse with their practice.
With the pedi we were with before, someone would take a message and normally the on-call doctor would call me back... but sometimes a nurse that wasn't in their practice.
I always get my pediatrician. Every single time I've called. It goes straight to her cell phone. Even at 2 in the morning. That's why I love her. She even saw her on a Sunday and she called our house to check up on her the next day.
We get a call back from the on call Pedi. They check their messages every hour and have had a great experience with them the 2 times we have had to call on a night or weekend.
Dx: PCOS and short luteal phase
18 cycles (3 with our RE) - Metformin + Clomid + HCG booster did the trick!

The practice we go to, it's one of the pediatricians from the practice that is on call and calls back. It makes it really nice because sometimes they will decide to call in a script to the 24-hr pharmacy so we don't have to take at trip to the ER.
It depends on what is needed. We get a nurse from the local children's hospital. They staff nurses just for answering these calls. If needed, they will connect me to my on-call pedi.

This is us, too. We leave a message and a nurse from TxChildren's calls us back. They triage the calls, so how long it takes depends on what the issue is. If they feel the situation calls for it, a doctor has called us back as well. If our pedi is on call, she'll call us back but I don't think she is/should be under any pressure to do so if she is not on call.
We hear from a nurse service (actual nurses). Our doctor has strict guidelines about contacting a doctor -- meaning she really wants them to in most cases.
I feel confident I can get hold of our pedi whenever we need her. She does not stand behind a big wall. Maybe that's naive on her part, but we respect her privacy in order to get access when we feel it is urgent.
Specifically, we have managed to get our doc's cell number and back line phone number in the past 4 years. Those are invaluable.
She hasn't changed the numbers, so we assume we're using them appropriately (like during the hurricane when her phones were out or when we were in isolation at the NICU and they wanted to change meds). If you can wing that kind of situation, it is peace of mind.
Our doctor partners her after hours care with several doctors. One of these actually made a house call for us with our first son.

As a PSA to newer moms and dads, any time you're unsure, ask to have a doctor call you. Or, go to the emergency room. No matter how good your doc, it is important to make your own judgment calls on these things when your child is sick after hours. When in doubt, go to TCH. Better to make a needless trip than to skip a trip you need.
