They started him at daycare, I just follow suit when I can. I bought some small plastic cups, and maybe fill them halfway. I have to keep reminding him to use 2 hands and drink slowly - they use Dixie cups at DC, which are much smaller, and I think he's a bit overconfident because of that. He's been doing it for almost a year now and has a good handle on it, but is by no means a pro.
We started with water and put in small amounts - like an inch or so and showed her how to hold the cup. Now she gets a full kids cup of milk with dinner and does a great job. But we still don't let her walk around with an unlidded cup - not yet.

We started with just a tiny bit in his cup and kept on refiling. He was moved to the 2 year old room at about 18 months at DC and they don't use sippies there. He got it pretty quick.
DD has gone to a Montessori daycare since she was 8 weeks old, and they began using open cups in the infant room (they don't allow sippy cups). We didn't really integrate it at home as much until she was about a year old or so. Since then, she has always used an open cup at home, though we do use sippies and take & toss straw/lid cups when we're out and about.
I'd say it took a couple of months for the frequent spills to stop (though she still has occasional accidents, of course). But we always put just a little liquid at a time in her cup, which helps. And we also use small, child size cups. We bought hers form daycare, but they're similar to this. She can handle larger cups as well (like the multi-color plastic ones from Ikea), but we like the smaller ones best.
She's great at drinking from a cup now, but I still usually start out with a cover and then she asks me halfway through dinner to take it off. And I don't let her have an unlidded cup walking around.
Sometimes she sticks her hands in it or drops food in it which drives me nuts.
He gets a cup of milk at the table and he can drink out of my glass, IF his mouth is cleanish. We just gave him a cup of water when he was about 18 months and let him have at it. He got the hang of it pretty fast. He only gets it at the table or in the kitchen though. He's been known to just pour it all out.
DS drinks out of cups almost exclusively...just uses straw cups for water when he's not at the table.
We just used small cups and started out only filling them 1/4 of the way. Sure, he spills sometimes, but we always just remind him to use both hands and pay attention. He does really well!

My DD has been using a cup since right before 2. I only let her use a regular cup at the table with 2 hands or with my help elsewhere. I started by giving her just about one mouthful of water. Then I would refill as she practiced. Once she was really good with it I started giving her more and more. Then I added juice. It was actually easier than I thought it would be.

I started with shot glasses with water in it.
