Has anyone done this? How did you work timing, etc.? We are looking to TTC#2 and had TTTC our first time. Got pregnant on IUI#2 with Clomid/Ovidrel. We're unexplained. Really concerned with having multiples (amongst other concerns, mainly financial). Thought unmedicated might be a good option. Other option is Femara instead of Clomid. Thoughts??

We did 2 unmedicated cycles. The reason it made sense for us is because we were dealing with MFI and the doctors (RE and urologist) thought we would benefit from just the boost that an IUI gives you. They didn't work, so we are now on a clomid + ovidrel cycle.
Did you have any side effects from Clomid the first time around? Is that why you think Femara might be a better option? I don't think Femara gives you less of a chance of multiples, unless you dont' respond as well and only produce 1 follicle instead of more than 1.
Clomid increases your chance of twins, but only by a few percentage points. So if the natural rate of twins is 1 in 10, on clomid it's like 1 in 6, but that's only if you produce more than one follicle. If you are being monitored (and you should be) your doctor can always cancel the cycle if you produce more follicles than you feel comfortable with.
No side effects from Clomid the last time. My RE suggested Femara if multiples was a concern, saying Femara has a lower risk of mulitples (Clomid=8-10% vs Femara 4-6%). I usually produced 2 follies on the Clomid. And of course I'll be monitored....I've been through this before!
One of the compounding factors they found the first time around was cervical stenosis with a difficult cervical tract. This should have been corrected with vaginal delivery, but there's a small chance it doesn't. Since I'm ovulating now (which I wasn't the first time around), thought unmedicated might be a good route...
How did you time the unmedicated IUI's? monitoring with ultrasound? OPK? Temping? just curious
Since you are ovulating and nervous about multiples, I'd definitely give the unmedicated IUIs a shot because if you end up producing 2 follicles on Femara as well (less likely, but definitely happens on here a lot) then I think you have the same risk of multiples as the clomid at that point?
I was monitored by my RE. So I'd go in on CD9 to do b/w and an ultrasound and then depending on how big my follicles were, he'd have me come in the next day or the day after to check again. When they detected the surge in my blood and the follicles were over 20 on the u/s, they would schedule the IUI for the next day. If I had to do it over again, I'd ask for the trigger shot because the timing is better that way. Some REs wait until you get a positive OPK, but I would have missed my surge both times if I did that so I'm glad he did it this way. I did temp, but just to confirm ovulation for my own knowledge. GL
