DS usually sleeps in a woombie. Tonight, I woke up bc he was grunting and it was right around his normal midnight feeding time. When I looked at him, he had his hands (still in the woombie) all the way on the top of his head so that the fabric covered his face up to his eyes. He looked like 1) a boohbah and 2) a suffocation risk. He was breathing fine, but now I'm worried to leave him in it. Any woombie users ever see this?

Sounds like maybe you should try the smaller size or the Houdini style of the Woombie. Hope it works out OK!

A boohbah is a super creepy children's cartoon thing. To me it looks like a cross between a teletubby and a scrotum.Here's a pic.
Have you tried a halo sleep sack? We just started using them and they are AMAZING!
They are a blanket sleep sack with velcro swaddler thingies for your LO's arms. Our LO ninja'd her way out of every type of swaddling we've tried until we tried the halow sleep sack swaddler.
It's great.
