GD Diet and ketones in urine

Hi Girls,

So I saw the Endocrinologist yesterday to get set up with the blood glucose monitor and to get a referral to a nutritionist.  The nutritionist can't see me until next week so in the meantime, I'm just going on what the Endo doc told me to do.  I woke up this morning at 3am starving so I checked my blood sugar so I could have a little snack and try to go back to sleep.  It was 53. 

After lunch/snack today, I checked my urine and it came back extremely high for ketones.  I'm a little confused what this means but I have a feeling it means I'm not eating enough carbs.  But if I eat carbs, my blood sugar goes over 120.  (after lunch it was 118- and my lunch only had quinoa for carbs).

Any insight would be appreciated.  I feel a little lost here and I just don't want to hurt the baby!

  • I got the ketone strips even though it isn't required for me.  What I did learn about ketones about a month ago when I had a 24 hour bug was that they can stay in your urine for at least a couple days, even if you are back to eating normally and getting enough carbs/nutrition.  In my case they were still around for almost a week b/c I couldn't keep down food or water for 18+ hours and that threw things out of whack.

  • when i was first diagnosed i was testing my ketones every morning.  EVERY morning for about 4 weeks i tested in the "high" levels.  i asked my dr about  this b/c i was eating everything that i was supposed as well as drinking plenty of water.  she said not to worry about it at all.  she actually told me to stop testing b/c it was irrelevant based on the fact that everything else was fine.  do what your dr tells you but don't lose sleep over it. and obviously call and ask if you need clarification.
  • So I go tomorrow to get put on insulin, but did the dr give you a little bit of a feeling of how to measure how many carbs you should be eating?

    My diet plan is 15g of carbs  = 1 carb point

    (you minus the fiber from the carbs so let say 19 g of carbs/ 4grams of fiber = 15 grams of carbs)

    Breakfast = 2 carb points / 1 protien

    Snack 1 = 1 carb point

    Lunch = 3 carb points/ 2 protiens

    Snack 2 = 1 carb point

    Supper = 3 carb points / 2 protiens

    Snack 3 = 1 carb / 1 protien

    The protiens help make the carbs last longer and not spike the blood sugar. Sometimes no matter what you do your sugar levels can not be controlled. I stick to a strict diet and yet I still have high over night numbers and high breakfast numbers and I had to be on insulin. My nurse / dr told me during my first pregnancy that I wasn't a failure or anything like that (I felt really guilty) but that sometimes that is what happens and it is ok..... some women just need the added insulin. HTH.

  • I was told to only test in the morning before eating.  
  • Are you having a nightly snack before you go to bed? And when you got the 53 - did you eat something? And if so - what did you eat? I don't know how much you know about GD but when pregnant - they don't like your blood sugars to go below 60 or above 120-130 after meals. And you do have to combine your carbs with protein and fiber. That definitely helps keep your levels down. Once you meet with your nutritionist - he/she will give you a diet to follow with a certain number of carbs per meal and snack. My plan is 45 per meal (breakfast, lunch and dinner) and 20 per snack (2x a day) with a nightly snack of about 35-40 carbs right before bed. I do 4 shots a day of insulin but my body is constantly changing during the pregnancy so the number of units of insulin I take is always changing.

     With ketones - getting a high number here and there (I've been told) is not a big deal. When you are consistently getting high numbers in your urine (and by the way - I check first thing in the morning) - I've been told that it means you are not eating enough carbs - and your nutritionist will just have to adjust your diet. It's nothing to worry about. It will get dealt with and resolved. You are already in good shape with an endo and nutritionist. They will definitely take care of you :) But don't forget to ask lots of questions and make sure they will be available for you to contact with questions when you leave the office.

  • Thank you all for the great advice!  I do have a bedtime snack I think I just need to eat more carbs in general (I have been terrified to eat too many since I got diagnosed). 

    You girls are the best!  It helps so much to hear from people who understand.  (((hugs)))
