Health Insurance w/maternity coverage

My current health insurance premiums are increasing by $200 a month this January.  Can anyone recommend a good company and plan?   I have a small business and my husband is employed by me. We can either do a family plan or a small group health plan.   We also have an HSA but we pretty much used up all our funds in there for fertility treatments, so we don't really care if we go that route again.  Any recommendations? 
TTC #1 since 9/07
Dx MFI, AMA, Endo, AMH .16
Lap 10/09 Removed endometrioma, stage IV endo and adhesions
Lap 2/10 Endometrioma cysts & adhesions returned.
Ivf #1 4/10 Antagonist, ET Cancelled.
IVF #2 2/11 A/ACP+E2V C/P
IVF #3 6/11 Letrozole/Antagonist BFN
IVF #4 11/11 Low stim Antagonist BFN
Lap 3/12 Lap & Selective HSG
Many cycles of Letrozole and LP HCG w/TI and LDN
IVF #5 8/12 Low stim BFN
IUI #1 10/12 BFN br> S&PAIFW

    I probably won't be much help by I have Federal Blue Cross Blue Shield.  I did not have any co-pays throughout my pregnancy with my appointments and only had a co-pay cost of $150 for myself and daughter during the delivery and hospital stay.

    We have our own individual plan through Highmark Blue Cross Blue Shield. We are on their direct blue plan. Our monthly premium is a little on the high side but it covers 90% of everything, and we have no co pays. It has been great, my youngest daughter has had health issues from the time she was 1 month old, and has been going to two different specialist, and the insurance has been great. I would highly recommend them

    I am thinking you probably already made your decision on this, but I am in the same boat as you.  Small business owner, using HSA.  We have a high deductible plan through Geisinger that costs us $493/mo for a family of 5.  We have a $5,700 deductible, but we fund the HSA in that amount each year through the business.  Well-child checkups, immunizations, my annual exam and other girly "things" are all covered 100%.  And it includes maternity coverage, which we probably won't be using.  We've had the plan for a little over a year and have had a $50 increase in premium so far.  I am very happy with everything so far.
