Back from DH's urologist appt and a ?

DH had his second urologist appt this am and I went with him. All of his labs, hormone levels, etc. have come back normal. They tested his urine after he ejaculated to see if maybe the sperm were getting backed up...both SA and urine came back with no sperm. So, we are going to go ahead with the testicular biopsy. The urologist said there's about a 33% chance that DH has an obstruction that can be fixed during the surgery. If they don't find an obstruction and just find very little or no sperm we will have to talk about our next step. My question is, has anyone been in a similar situation and had success with removing the blockage? Once the blockage was removed, did DH's sperm count become normal? Since DH had one testicle removed d/t a torsion and all his bloodwork is ok, we are really hoping that it is a blockage and can be fixed and the blockage occured when DH had his surgery for the torsion. I wish the diagnostic testing was over so we could have a definite plan!

    My DH didn't have a blockage and they found no sperm during the biopsy.  The biopsy was what gave us our final dx of sertoli cell syndrome.

    I hope you have better luck than we did and they find some sperm.  Lots of PPVs for you and your DH.

  • Will the uro do an u/s?  DH had an u/s in order to confirm no blockage so now were are talking about biopsy or TESE.

  • I have another question... did your uro explain why he is moving onto biopsy instead of TESE?  We have an appt today and our uro seems to be pushing TESE. I am going in with so many questions, I don't think this doctor knows what he is in for.

    Our uro didn't mention an ultrasound to dx a blockage...maybe we should call and ask about that! Forgive me, but I'm relatively new to all this, but isn't TESE used to retrieve sperm when going through IVF to make the embyos? We are still at the diagnostic stage, and if the uro does find sperm he'll have to go through another procedure when we decide to do IVF to extract the sperm at that time. Does that make any sense? I may be way out in left field...the world of infertility is so complicated. Good luck with your appt today.

    Thanks!  We had one SA come back with a few sperm so that might be why the uro isn't doing a biopsy for DH.  We are no where near ready for IVF and we are pretty sure that we will do the TESE at the same time.  You are right about what a TESE is, it will *hopefully* retrieve enough sperm. 

    I am going to ask about a biopsy, I just don't want DH to have multple procedures.  A biopsy can retrieve sperm also but will mostly confirm the reason for the azoo.

    Thanks! Are any of your DH's labs abnormal? I just can't figure out why all my DH's labs are ok and hormone levels are ok, but he's not producing any sperm (or they aren't comming out)!

  • DH's hormones are sort of a mess.  Hypothyroid, low FSH, and low testosterone.  He is on synthroid for his thyroid and I am asking the uro about testosterone therapy.  I don't know what role the hormones play just yet but hopefully I will know more later.

  • My husband is getting his preliminary blood work done tomorrow for the biopsy...  We don't have a date yet on the biopsy. 

     

    My DH SA came back AZOO  both times.

     

    Everything else but FSH was normal...FSH was at 17.5  

    I think the reason we are doing the biopsy is because it will get to the bottom of where/if any sperm are...and if not, they have the tissue to find out why the azoo exists...

    The FSH isn't  horribly high, but it is elevated.  I have heard some RE's don't read to much into it...BUt then I have heard that high FSH indicates a testicular failure of some sort...

    As soon as we move further along- I can let you know kind of what happens atleast with us.

     Find out what RARE means.

    We didnt even have that DH's SA.

    It has been extremely hard for both of us...
