Fussy while bf'ing all of a sudden?

DD started becoming fussy about a week ago while nursing from only one side.  Now for the past 2 days, it's both.  She appears content and perfectly happy afterwards, as if she's getting enough.  When I pull her off for a nipple break, I can squeeze and milk comes out.  She starts out normal then about 5-10 minutes into it, she yanks her head around and damn near pulls my nipple right off while kicking her legs really hard. 

I've never been able to feel a letdown so I don't know if it's coming slow or fast during this time.  I have noticed, while pumping at work, that I'm getting more foremilk than hindmilk (I think!).  Imbalance?  She's taken a bottle, no problem, since her first week when we had to supplement with formula for a few days due to jaundice.  I've been back at work for about 3 weeks now.  She takes 1-3 bottles while I'm gone, depending on my shift.  Prefers bottle?

What gives?!  Thanks!

    SAME thing has been happening with DS. It's SO frustrating. It happens mostly in the evenings and sometimes he will just SCREAM. I'm pretty sure I have a slow let down and he's getting impatient. I also think the 3 month growth spurt is making it worse.
