I am not very good at this either but I have been drinking a glass of V8 Fusion in the morning and a glass of the regular spicy V8 in the afternoon. I also try to remember to bring carrot sticks with ranch or humus to work to snack on.
i take juice plus every morning. it saves my life since i can hardly stomach veggies since i've been preggo. it also helps my cholesterol, so there you go!
It was hard for me to eat veggies during the 1st trimester and I am vegetarian. Now, I drink v8 during the morning (with prenatals), veggie sticks with hummus, celery with peanut putter (I dislike celery, but can eat it with pb), veggie soups (especially with fall upon us).
I add a veggie puree to almost everything we eat for dinner... spaghetti sauce, soups, and casseroles all do really well with some extra veg. I usually have pb&j for lunch, and I add extra berries to that. I used to do fruit smoothies a lot (frozen banana, handful of frozen fruit, juice, yogurt & protein powder + blender) but bananas are a casualty of the Kid. Also, think ethnic... Indian food in particular is really good for including a lot of veggies.
I'd suggest you try meal planning... I find it really easy to keep on top of all my nutrition stuff. Today we had black bean quesadillas and chili lime corn, and because I knew it was coming it wasn't nearly the pain in the ass it could've been.
My favorite way of adding extra veggies is through tomato sauce - steam some broccoli, carrots some onion...etc...italian seasonings and blend with 1 can of tomato sauce and 1 small can of tomato paste and voila! ...you have a hearty healthy tomato sauce

I also will do things like stuffed shells and add zucchini and spinach to the filling.
Zucchini or carrot muffins stuff like that.

I eat a lot of leftovers for lunch, so as long as I am making dinners with lots of veggies, I usually get them during the day. Some examples are:
I add double veggies to dinners like Chicken Pot Pie and Soups (a plus is it adds bulk also, so it lasts longer for leftovers)
I add corn/peppers to anything mexican (enchiladas, soft tacos, etc)
I dump an extra can of diced tomatoes (or diced fresh tomatoes) to my spaghetti sauce
I add a package of frozen spinach to stuffed shells/baked ziti
I add broccoli to mac n' cheese
For breakfast, I will put green peppers in my eggs (or whatever veg I have in the fridge)
If I have a sandwich at lunch, I add lettuce, tomato, and cucumber
