I am going to call the pedi this morning to see what they think but Elli has the single worst diaper rash that I have ever seen in my life. It hurts me just to look at it so I can't imagine how she is feeling.
My question is how do you know when it is diaper rash or yeast? We have tried about 4 different diaper rash lotions and none of those seem to be working as a matter of fact it seems to be making it worse. I guess to answer my own question maybe that is an indication that it could be yeast. It is also up towards the front of her and not the back along with some pretty nasty BM's several times a day.
This is her first diaper rash in 13 months and it couldn't be as simple as just using diaper rash lotion could it...nope had to be difficult.

From what I understand, all diaper rashes are yeast based? When Jake is on antibiotics or has a stomach bug/diarhea, he gets bad rashes. I've never found a diaper rash cream that works on it's own. They only way I can get them cleared up is to use a combination of antifungal cream & hydrocortizone. Then I layer the diaper rash cream on top for extra protection. You can get the antifungal cream that is sold for athlete's foot.
If you are going to the pedi for this, see if you can get an RX cream. It will have all of these things + something to numb the skin a little bit. It's nice to have on hand for the future too.

jocelyn occasionally gets a very "beefy red"-appearing rash that looks like a burn. usually this is associated with PUC (poop of unusual consistency) that is typcially copious. It also seems to affect her frontside more than her backside. this is a "yeast" rash for her, and does not improve with regular diaper cream. We have to use Nystatin on this rash which works great but requires a prescription. We were always taught in med school that yeast has "satellite lesions" or little bumps that spread out from the main rash, but this hasn't been our experience.
Love the stuff! What's really nice is that you can take it to daycare as an RX. Otherwise, you'd have to ask the DCP to put on three creams, which might be a bit much. I didn't know that's where the name came from!
Since you said it's more toward the front and not on her bottom, I would say it sounds like yeast. That is the way it was when Emma had it. They called in an rx for nystatin and it took about 5 days to be completely gone (in case they don't tell you that, like they did me!).
As for regular really bad diaper rash (we get it from sharing too many grapes with daddy and corn on the cob) we use motrin and calmoseptine. I will say that on a really bad one, the calmoseptine burns a little until you get the diaper on. There is menthol/calamine in it and Emma will cry, but as soon as I get her diaper closed and push on her booty, she feels better. I'm guessing it's the cooling effect with the air hitting that scares her/burns a bit.
