Milestone charts for the baby book?

Our baby book doesn't really have many of the milestones listed in it. I'd like to find something that would work well to put into the baby book that lists the different milestones with some sort of checklist to put the week she did them next to it.

Any ideas?

if nothing else, could you print out what you want it to say, and then put it on some cute scrapbook paper and then put it in the book?
