Breastfeeding

2 months old- slow weight gain

In the beginning dd was gaining really well, but it's started to drop off a lot.

for the past 2 weeks she's only gained 4 ounces a week. We saw the pedi last week and she said 4oz/week was fine.  I've read that 4 ounces isn't enough, but since pedi said it was fine I said ok.

Well this week dd has only gained an ounce. We go back to the pedi on Friday for a weight check.  

My supply is fine, dd has plenty of wet/poop diapers, but she's still not gaining.  What can I do?

fwiw- she has reflux and I'm dairy and soy free because it seems to cause her problems.

 Does anyone have any advice? It's really starting to stress me out.  I hate to even ask, but would formula help her gain?

  • I can't really offer any specific advice but I can tell you that my DD was only in the 23% for weight at her 2 mo. check up.  Our pedi wasn't concerned because she was steadily gaining weight.  After observing her for awhile, he also said that the reason she wasn't gaining more quickly is because she never stops moving.  She's always kicking or moving her arms, etc. so she's just burning through calories really fast.

  • DS is gaining slowly, too.  We actually have a weight check later today.  In my case, I think it was partially due to a supply issue.  I've worked the last 1-2w to get it back up and finally noticed a difference about 2 days ago.  If your supply is ok, I don't think formula would help since it has a couple fewer calories per oz.  Our pedi recommended we give DS an ounce or so of EBM after each feeding.  It's been a mixed bag - sometimes gulping it down, other times spitting it all back up and sometimes refusing it.  We'll find out today if it helped, I guess.
  • My DS is 6-weeks old and after losing weight for the first couple weeks, he is finally on the rise.   My LC says that my supply is fine and that the issue is that DS is not getting enough hindmilk when he nurses.  She just recommended nursing for as long as I can get DS to eat on one side and to use breast compression.  So I'll nurse DS and when he nods off or pulls off, I'll burp him or change his diaper and re-offer the same breast.  He is still a slow nurser--sometimes he'll spend 45 minutes or more on the one side.   I sometimes feel like we fight for every ounce he gains, but he is averaging about half an  ounce a day now and my pedi and LC are happy with that.  
  • I would also suggest nursing longer on one breast to increase her hindmilk intake - that's where the fat is.  If you have a really good supply, she may be getting mostly foremilk if you are switching sides - that will give her plenty of wet and dirty dipes but make her slow on the weight gain.
