feeding/diaper template?

Hi, I'm trying to get things in order before bringing baby home, and I was wondering if anyone had a good template or knew of a good website with a printout to monitor/record daily feedings and diaper changes for baby?  I could probably make one, but I am completely out of creativity!!  Thanks!
