I am 4 months pregnant and my breasts have still not grown! I know I should register/buy some nursing bras soon but still have no idea what size to buy. Do your breasts continue to grow until the baby is born or is this the size they will stay? Should I wait until the last minute to buy them to see what size I will be? Or is this as big as they're going to get?

Honestly don't buy anything until you have your LO. If you do want something for the hospital buy a Medela sleep bra...even if it's a little big or small it will still work. I wore one to bed every night for the first 4 months because I had to wear breastpads for leaking. You could also buy a nursing tank...buy it a little big in the bust area.
Some people will grow 3 cups sizes while others grow none so it's hard to say what you will do. You won't know what yours will do until your milk comes in a few days after your LO is born. Once that happens you'll be engorged for a week or so and you'll probably be bigger than what you'll actually end up. Good nursing bras will run you anywhere between $50-$70 so I think you should just hold off for now. If you can just live in nursing tanks and sleep bras for the first couple of weeks after your LO is born.
Some will say you can buy them during the last four weeks of your pregnancy. I was worried that mine would get bigger, so I waited until after DD was born and my milk had come in. Honestly though, mine are the same size they were at the end of my pregnancy, so it probably would've been fine to buy them towards the end of my pregnancy.
I did buy some nursing tank tops and night nursing bras before she was born so I could have those at the hospital and when we were first home. It's nice to have those in the hospital.
I haven't had my baby yet, but I had a fitting at a boutique by a certified LC where they guarantee the bras they fit you for. If they don't fit after the baby arrives, you can exchange them for the right size. I figured that they wouldn't guarantee them if they didn't have a really good idea of what your body will do, and they wouldn't still be in business after 5+years if they weren't great at what they do. They told me that after you pass 20 weeks, your cup size will not increase until your milk comes in (and then it will regulate itself back down to your cupsize at the end of pregnancy). Because your ribcage continues to expand, your band size will likely go up, but once you have your baby, it will start to go back to what it was.
For the first few weeks while you are engorged, you will wear mostly sleep bras and nursing tanks. I bought three of each of these. Once your milk regulates a bit, you can wear a more traditional nursing bra (no underwire, it can cause clogged ducts and mastitis). I bought a couple of these as well. The key here is to make sure the band is a great fit - that is where all of your support will be coming from. I would recommend getting fitted, even for the sleep bras and tanks. If you are larger chested (I am a 34dd/e), I highly suggest trying bravado. They are a tad bit more pricey, but will provide great support.
