I think 6 months is usually the time to up the size according to the manufacturer...we recently switched to size 3 with Dr. Browns. s

I'm sure there's more specific answer, but I just follow off of DS's lead. He started to get mad during feedings, so i switched sizes and he was a much better eater. He just wasn't getting it fast enough.
Not a dumb question at all. It's definitely NOT an exact science. If you think your lo might be ready to move up, go ahead and give it a try. If they don't choke, they are probably just fine!
She's still using the slow flow but sometimes she chokes because it's too fast for her. I was afraid that the medium flow would be even worse or is my thinking wrong?
Are you nursing at all these days? I know you were talking about going to formula, but I can't remember if it was 100% or only partially. If you're nursing at all, it's probably best not to change the nipple size so that she won't get frustrated at the breast. If you're not, I agree that you should probably just follow her lead. If neither of you are frustrated with how long it takes her to finish a bottle, then I say leave it be.

We moved up from stage 1 when she was sucking so hard that the nipple collapsed (~3 mo.). We moved up from stage 2 when we were visiting my ILs, were short a nipple, and could only find stage 1 and stage 3 at the only major store around for 50 miles.
I still give her stage 2 when she's tired, and stage 3 otherwise because she's an impatient little eater.
