Any whos LO has stains on their teeth

DD has these reoccuring stains on her teeth.  She had a cleaning a couple months ago but insurance will not cover another for 6 months so I am wondering of any helpful hints on treating these stains at home.  Anyone dealing with this?

I can not figure out what they are coming from.  From what I can tell the top causes are bottle which she has not been on since she was 13months old, iron which she in not on and flouride which I took her off of.  I feel like it got worse after I stopped the vitamins.

The dentist says its not hurting her teeth just looks bad!  But I can not stand them
