Reserching Day Care

I am wanting to go the in home day care route, and we have found someone based on recommendations of co-workers.  I already know that she is state certified, what she does in case she's sick, and her hours.  I know that in years past folks had a list of questions to ask, but of course I don't have those questions.  Any suggestions on things that I need to ask/look for would be most appreciated.

    how often does she clean/santize?

    cpr certification?

    policy regarding a sick kid?

    fees if you decide to take a vacation?  do you need to pay a fee to hold a spot open?

    what will you need to bring for the baby?  (I had to purchase five extra crib sheets for daycare purposes)

    are multiple age groups together?  example:  are three year olds able to have open access to newborns?

    transportation - will she be taking the kids with her to run errands and such?
