One game I played recently at a shower (it was a co-ed shower, but could work with all women too) was the change a baby relay race. You get into 2 lines of equal amounts of people and there is a baby doll for each team on the table and a diaper beside the baby doll. Each person has to change the baby's diaper. The team who does it the fastest "wins".
It is pretty funny with people who have never changed a diaper before.
Give each guest a birthday candle, strip of paper, and ribbon. Ask them to write a wish, blessing, or special thought on the paper. The paper is then wrapped around the candle and tied with the ribbon. Collect all the candles and put them in a box for the mom-to-be. Every year on baby's birthday, one candle will be pulled out and placed on the cake. The wish/blessing/prayer can be read right before the candles are lit.
This was done at one of my showers, and I can't wait to pull one of the candles out for DD's first birthday. I think we have 18 candles, so DD will have many year's worth of reminders that she was loved even before she was born.
when guest arrive give them a plastic baby clothes(diaper) pin that they pin to their shirts, and tell them that they cant say the word "baby" who ever says it has to give it to the person who calls them out, at the end of the shower, who ever has the most clothes pins WINS!!
I JUST went to a shower this weekend and they had us design our own onesies! Not exactly what your asking for but I thought I'd share it.
They had a table with stencils, fabric markers and fabric paint and some plain white onesies. Everyone was able to paint & design their own onesie for the baby to be!
It was actually a lot of fun! Just dont forget to have the hand wipes nearby, it can get a little messy.
we did two different toilet paper games... lol they are cheap and fun!
first one is kinda like the sring game previously mentioned- guess about how many squares mommy is aorund the biggest part of the bump, winner gets a prize
second is best toilet paper diaper- hilarious- have guests pair up- one person wears, one person wraps. the person who wraps get on roll of toilet paper to make the best "diaper" they can on the person who wears. the person who wears has to run 20 feet in the diaper and back to the starting point. best diaper in terms or looks and durability after the run wins a prize

We did the string game. Were everyone has to cut a piece of string to what they think is the circumfrence of the mommy to be's bellie is.
Then we did a list of what attributes that the mommy wants the baby to have. and whoever had the most right won
